支持性心理护理在耐多药结核病病人的临床应用与研究

摘　　要：目的探讨支持性心理护理对耐多药结核病患者的心理干预,并建立耐多药结核病患者心理护理方案。方法将60例在我院治疗的耐多药结核病患者,随机分为研究组和对照组,对照组给予常规耐多药结核病治疗和护理;研究组除常规治疗外,另加支持性心理护理,分别在治疗前后进行生活质量综合评定问卷-74(成人用,GQOLI-74)进行调查评定和干预前后自卑心理、紧张恐惧、自叹自怨、怨天尤人、混合型的心理表现改善情况比较。结果患者治疗前两组GQOLI-74评分无明显差异(P>0.05),而治疗后两组评分则有明显的变化,研究组优于对照组(P<0.01);同样心理测试改善程度治疗前两组无明显差异(P>0.05);治疗后研究组明显较对照组行为改善(P<0.01)。结论支持性心理护理有利于耐多药结核病患者的心理康复和行为改善,提高耐药结核病患者的依从性和疗效。Objective To study the psychological intervention of supporting psychological care in patients with multi-drug resistant tuberculosis（MDR-TB）,and to establish psychological care programs for the patients with MDR-TB. Methods Sixty cases of MDR-TB treated in our hospital were randomly divided into study and control groups. The control group was given the conventional MDR-TB treatment and care and the study group was given supportive psychotherapy and care besides conventional treatment. Before and after the treatment,comprehensive assessment questionnaire-74 of life quality（GQOLI-74 for adults）was used to investigate pre-and post-intervention inferiority,tensian,fear, fretful complaining,a mixed-type situation,etc. Results There was was no significant difference in GQOLI-74 score before treatment between the two groups（P〉 0.05）,and there were more changes in the score after treatment in the study group than that in the control group（P〈 0.01 ）. There was was no significant difference in the improvement degree of psychological test before treatment be tween the two groups（P〉 0.05 ）, and there was more improvement in the degree after treatment in the study group than that in the control group（P〈 0.01 ）. Conclusion Supporting psychological care is in favor of the psychosocial rehabilitation in MDR-TB patients and help to improve their behaviors and compliance as well as treatment efficacy.
