摘 要:目的对比研究波动性高浓度葡萄糖与恒定性高浓度葡萄糖对体外培养的人脐静脉内皮细胞(humanumbilical vein endothelial cell,HUVEC)的凋亡,调控基因bax、bcl-2的表达及血管舒、缩因子一氧化氮(nitricoxide,NO)、内皮素-1(endothilin-1,ET-1)合成的影响。方法体外培养HUVEC,实验性模拟糖尿病的血糖波动状态,实验分组为,正常对照组(NG组)(葡萄糖浓度为5.55 mmol/L);恒定高葡萄糖组(HG组)(含葡萄糖25 mmol/L);波动性高葡萄糖组(FG组)(葡萄糖浓度为5.55与25 mmol/L2种条件培养液);每间隔8h更换新鲜条件培养液1次,共作用72h。流式细胞仪检测细胞凋亡率及相关调控基因bax、bcl-2表达,硝酸还原酶法测定内皮细胞合成的血管舒张因子NO含量,放射免疫分析法测定内皮细胞合成的血管收缩因子ET-1含量。结果 FG组细胞凋亡率、凋亡基因bax表达[以荧光指数(fluorimetric index,FI)表示其相对含量]以及ET-1合成明显高于HG组,分别为(10.20±0.20)%比(8.10±0.10)%(P<0.01);1.250±0.048比1.120±0.010(P<0.05);(3.50±0.50)pg/μg Protein比(2.70±0.50)pg/μg Protein(P<0.05),而FG组细胞抑凋亡基因bcl-2表达(以FI表示其相对含量)、NO合成明显低于HG组,分别为,0.790±0.062比0.900±0.059(P<0.05);(13.50±2.00)μmol/L比(18.10±3.70)μmol/L(P<0.01)。结论高波动性的血糖可能比单纯稳定的高血糖对血管内皮的损伤更为严重。Objective To compare the effects of intermittent and constant high glucose media on apoptosis and the production of nitric oxide(NO) and endothilin-1(ET-1) in cultured human umbilical vein endothelial cell(HUVEC). Methods Human umbilical vein endothelial cells were cultured to confluence and were exposed for 72 h to 5.55 mmol/L glucose normal control,25 mmol/L glucose constant high glucose and 5.55 mmol/L alternating with 25 mmol/L glucose intermittent high glucose on a daily basis. The cell apoptosis rate and the associated gene expression (bax and bcl-2) were detected by flow cytometry; The production of NO was measured by nitrate reduction test, and the production of ET-1 was measured by radioimmunoassay(RIA). Results The intermittent glucose group had increased, the apoptosis rate,bax(expressed by the fluorimetric index,FI) and ET-1 compared with constant high glucose group,(10.20±0.20)% vs (8.10±0.10)±( P〈0.01);1. 250±0. 048 vs 1. 120±0. 010( P〈 0.05) ; (3.50±0.50) pg/μg protein vs (2.70±0.50) pg/μg protein( P〈0.05). However, both the expression of bcl-2(expressed by the fluorimetric index, FI) and the production of NO in intermittent high glucose group were significantly lower than those in constant high glucose group,0.790±0.062 vs 0.900±0.059(P〈0.05),(13.50±2.00) μmol/L vs (18.10 ±3.70)μmol/L( P -40.01). Conclusion These findings suggest that variability in glycemic control could be more deleterious to endothelial cells.
