不同孕龄母-胎血清白喉破伤风抗体对照研究

摘　　要：目的揭示不同孕龄胎儿血清抗体与母体血清抗体的关系,掌握不同孕龄胎儿白喉、破伤风免疫力(抗体)的自然消长规律,为白喉和破伤风疫苗的预防接种提供理论依据。方法①采集不同孕龄引产、早产、正常产孕妇的母-胎血清,根据不同孕龄分为5组,详细调查血清采集对象的既往史、疫苗接种史;②将采集的母胎血进行血清分离、编号、冷冻、保存;③进行母-胎血清抗体测定。结果在检测的5个不同孕龄组中,胎儿白喉和破伤风抗体与母体白喉和破伤风抗体有明显的相关性;各组母胎间、母体间、胎儿间白喉、破伤风抗体阳性率和母体抗体几何平均倒数浓度差异均无统计学意义(P>0.05)。结论进一步加强孕妇免疫状况的监测,实施孕前期人群的相应疫苗强化干预。Objective To explore the relationship between the fetal serum and the maternal serum antibody of different gestational age, and master the natural dynamics of the birthsdipbtheria,tetanus immunity （antibody） of different gestational age, and provide a theoretical basis for the diphtheria and tetanus vaccination. Methods ① the pregnant women-fetal blood samples of labor, premature birth, mother of the normal production of different gestational age were collected. According to gestational age, the pregnant women were divided into five groups. the detail previous history and the vaccination history of the serum collected objects were investigated. ②the mother-fetal blood were separated,matched numbers and eryopreserved. ③the mother-fetal serum antibody were detected. Results In the five different gestational age groups, There was a obvious correlotion between each group of fetal and maternal births-diphtheria, tetanus immunity （antibody） There were no significant differences of diphtheria and tetanus antibody-positive rate and the geometric mean reciprocal titre（ P 〉0.05） among the groups of maternal-fetal,maternal and fetal. Conclusion Strengthen the monitor of the immune status of pregnant women, and enhance the intervention of the corresponding vaccine of the pre-pregnant period.
