复方接骨胶囊对胸腰椎骨质疏松骨折椎体成形术后的影响

摘　　要：目的:探讨复方接骨胶囊治疗胸腰椎骨质疏松骨折椎体成形术后疗效。方法:对66例胸腰椎骨质疏松骨折椎体成形术后随机分2组观察,治疗组口服复方接骨胶囊每次3粒,每日3次口服。对照组服用迪巧维D钙咀嚼片,每次2片,每日1次,共观察4个月。观察临床症状、X线片和CT的骨折愈合、骨密度(BMD)的变化并进行对比。结果:止痛效果治疗组总有效率100%,对照组总有效率80%,两组总有效率比较,差异有显著性(χ2=13.344,P<0.01)。骨折愈合效果比较:治疗组总有效率100%;对照组总有效率70%。两组比较,差异有显著性(χ2=14.915,P<0.01)。平均愈合时间比较:治疗组临床愈合时间148-228d,平均(197.0±7.3)d;对照组临床愈合时间216-398d,平均(306.0±7.2)d。两组比较,差异有显著性(t=12.121,P<0.01)。相临椎体骨折:治疗组无再骨折发生,对照组发生3例再骨折,均为下位椎体。治疗前后BMD比较:治疗组治疗后BMD显著升高,对照组有所升高,两组比较有显著差异(P<0.05)。结论:复方接骨胶囊能够明显缩短骨愈合时间。可以预防骨水泥周围的局部骨吸收以及全身骨量丢失,从而进一步缓解临床症状和降低了相临椎体骨折的发生率,并且服用方便,毒、副作用小,是辅助治疗胸腰椎骨质疏松骨折椎体成形术后的有效中药制剂。Objective：To evalue the clinical effect of Fufang Jiegu Capsule on thoracolumbar osteoporosis fracture undergone percutaneous vertebro-plasty. Methods： 66 patients of thoracolumbar osteoporosis fracture undergone percutaneous vertebroplasty were divided into treatment group and control group at random. The treatment group was given oral Fufang Jiegu Capsule three capsules tid. Control group was given oral two Diqiao Vitamine D Calcium Chewing Tablets 2 pills per day. Treatment course was 2 months. Clinic symptoms, bone healing in x-ray and CT, bone mineral density （BMP） of two groups were observed. Results： There was significance of pain relief in two groups（χ2=13.344,P0.01）, the ratio of pain relief was 100% in treatment group and 80% in control group. There was significance of bone healing in two groups（χ2=14.915,P0.01）the ratio of bone healing was 100% in treatment group and 70% in contral group. Clinical bone healing time was 148-228 days （averaged 197.0±7.3days）in treatment group and 216-398 days （averaged 306.0±7.2days） in control group. There was significance of bone healing in two groups（t=12.121,P0.01）.There were no consecutive vertebral bodys refracture in treatment group and there were 3 vertebral bodys refracture located in next vertebral body in control group. There was significant increase in bone mineral density （BMP） in treatment groups than that in control group after treatmeat（P0.05）. Conclusion： Fufang Jiegu Capsule can significantly decrease bone healing time and prevent bone loss of body and local bone resorption arounding the bone cement. Therefore, Fufang Jiegu Capsule can improve clinic symptom and decrease the radio of refracture of consecutive vertebral bodys. Fufang Jiegu Capsule had the virtue of less side effect and convenient to use, so it was the efficient assistant Chinese medicine in treating thoracolumbar osteoporosis fracture undergone percutaneous vertebro-plasty.
