摘 要:目的评价后路短节段内固定器Stefee钢板治疗脊柱不同损伤程度提供的生物力学稳定能力及前纵韧带在Stefee钢板中的作用。方法利用传感器及电测技术,采用新鲜尸体胸腰段脊柱标本,计算平均顺应系数(fs)。结果Stefee钢板能恢复各种损伤的轴压、旋转稳定性,提高前屈、侧弯稳定性,前纵韧带断裂降低前中柱损伤后伸、侧弯力学稳定性,三柱损伤后伸失稳。结论推荐Stefee钢板治疗前中柱损伤,前纵韧带对Stefee钢板有明显力学意义,结合外固定可治疗三柱损伤。Objective To study the three dimensional stability of spinal injuries with different degrees treated by Steffee's transpedicular screws and plates (Steffee) with anterior bone graft.Methods Fresh cadaveric thoracolumbar specimens were used. They were measured by 3 degree of freedom electrogoniometer force and displacement transducers. The mean flexibility (fs) was computed. Results In anterior and middle column of spine, the plate can provide good stability in compressive loading, flexion, extension, lateral bending and rotation ( P <0 05); Cutting the anterior longitudinal ligament can significantly decrease the mechanical stability of extension and lateral bending of destroyed spinal construction ( P <0 05); In three columns destroyed spinal construction, the extension is unstable ( P <0 05). Conclusion Steffees posterior device can increase the mechanical stability of the injured spine in anterior longitudinal ligament, which is suitable for three columns destroyed spinal construction with external fixation. Firm bone graft is the basis of good mechanical stability of the plate.
