腰椎滑脱后路椎体间植骨融合

摘　　要：目的探讨经后路椎体间植骨融合治疗腰椎滑脱的疗效。方法采用Ｓｔｅｆｅｅ手术复位、固定后，从两侧进入椎间隙凿深２０～２５ｍｍ、高１０ｍｍ的骨槽，取髂骨块植入。结果８例随访７～４２个月，全部症状缓解，恢复轻体力劳动。术后６个月Ｘ线检查示植骨块无吸收，已融合。结论经后路椎体间植骨，一次完成椎管减压、复位、固定和融合，优于前路椎体间植骨术。植骨部位在脊柱承重轴上，融合后稳定性好。Objective The curative effect of spondylolisthesis treated by intervertebral body fusion with bone graft through posterior approach was investigated. Methods After being reduced and immobilized with Steffees instrument, the intervertebral ambilateral bony channels of 20-25 mm deep and 10 mm high were made with bone chisel, and then the iliac grafts were planted to the bony channels. Results 8 cases were followed up for 7￣42 months. They were able to do less strenuous physical work that they could do before the operation and the symptoms relieved. The X ray examination at the sixth month after operation showed that the bony grafts unabsorbed and the intervertebral body fusion formed. Conclusion The method that could decompress the spinal canal was better than that through anterior approach. As the region of fusion at the axis bearing the weight of spine, this fusion was more stable, conforming to biomechanical principles.
