^(18)F-FDG PET/CT对胆囊癌分期、治疗后再分期诊断的价值

摘　　要：目的评价18F-FDG PET/CT对胆囊癌分期、治疗后再分期诊断的价值。方法15例经临床检查初步怀疑为胆囊癌患者行全身18F-FDG PET/CT分期诊断,并在2周内完成手术或穿刺病理检查。胆囊癌术后和(或)放化疗后PET/CT随访17例。结果15例病理证实为胆囊癌,SUVmax均值为9.57±3.16,对胆囊癌的诊断正确率100%(15/15);局部淋巴结转移的检出率为81%(9/11);治疗方案改变40%(6/15)。随访17例胆囊癌术后和(或)放化疗后患者,13例PET/CT阳性并证实局部和(或)多发转移;4例显示阴性,其中2例随访1年后复发。结论F-FDG 18PET/CT对胆囊癌分期、治疗后再分期诊断有较高的临床价值。Objective To investigate the value of F-fluod- 18 eoxyglucose（^18F-FDG） PET/CT in staging and restaging diagnosis of gallbladder carcinoma. Methods The total of 15 patients potentially gallbladder carcinoma and other 17 patients with surgery and chemoradiotherapy followed up were included in this study. A whole-body ^18F-FDG PET/CT was conducted and staged 18 for increased focal uptake. Accuracy was determined by comparion of PET/CT with surgical staging or acupuncture in two weeks. Results The primary tumor was visualized in 100% （15/15）with PET/CT.The maximum standard uptake mean values（SUVmax）in the lesion were9.57±3.16 .For simple regional lumph nodes the sensitivity was 81%（9/11）.40%（6/15）of patients were facilitated in the treatment planning,other 17 patients with surgery and chemoradiotherapy followed up,PET/CT negative were showed in 4 and relapae in 2 cases 1 years after following up.The single and/or multiple metastases were found in 13 patients.Conclusion ^18F-FDG PET/CT imaging may be useful in the diagnosis and staging of gallbladder carcinoma and following-up patients with recurrent gallbladder carcinoma.
