孕妇乙肝病毒复制状态与新生儿HBV感染的关系

摘　　要：[目的]探讨HBsAg阳性孕妇HBV复制状态与新生儿HBV感染的关系。[方法]以117例HBsAg阳性孕妇及其所生的118例新生儿为研究对象,检测孕妇分娩前、0月新生儿、1月新生儿血清的主要乙肝标志物。[结果]出生时,母亲HBeAg阳性组新生儿HBV感染率(15.3%)高于母亲HBeAg阴性组(0%),母亲HBV-DNA≥104(copies/ml)组新生儿HBV感染率(18.0%)高于HBV-DNA﹤104(copies/ml)组(3.7%),差异均有统计学意义(P﹤0.05)。1月随访时,母亲HBeAg阳性组新生儿HBV感染率(9.7%)高于母亲HBeAg阴性组(4.3%),母亲HBV-DNA≥104(copies/ml)组新生儿HBV感染率(10%)高于HBV-DNA﹤104(copies/ml)组(7.4%),但差异均无统计学意义(P﹥0.05)。[结论]母亲HBeAg阳性、HBV-DNA≥104(copies/ml)是新生儿出生时发生HBV感染的危险因素,OR值分别为(OR=1.180、OR=5.707)。[Objective] To explore the effect of HBV replacation status of HBsAg-positive pregnant women on neonatal infection. [Methods] 117 HBsAg-positive pregnant women and their 118 newborns were choosed as our subjects. The venous blood of pregnant women before delivery, newborns at the birth, and newborns at the age of one month were collected to detect their major HBV serologic markers. [ Results] At birth, HBV infection rate of newborns with HBeAg positive group （ 15.3 percent） was higher than that in the mother with HBeAg-negative group （0%）, neonatal HBV infection rate in mother HBV- DNA ≥10^4 （copies/m[） group （18.0 percent） was higher than that in mother HBV-DNA 〈 104 （eopies/ml） group （3.7%）, the differences were statistically significant （P 〈 0.05）. At the time of one month, neonatal HBV infection rate （9.7%） of the mother HBeAg positive was also higher compared to that of the mother HBeAg-negative （4.3%）, and the rate of HBV-DNA t〉 104 （copies/ml） group （10%） was larger than that of HBV-DNA 〈 10^4 （copies/m[） group （7.4%）, However, the difference wAS not statistically significant （P〉 0.05）. [Conclusion] HBeAg-positive and HBV-DNA ≥10^4 （copies/ml） of pregnant woman were both the risk factors of their neonates at birth infected with hepatitis B virus （OR = 1.180, OR = 5.707）.
