摘 要:目的:建立猪胸腰椎爆裂性骨折B型模型。方法:收集猪T13-L3节段标本30例,随机分为3组:终板预损伤组、L1椎体预损伤组和不做预处理组。采用预损伤后逐级撞击法撞击标本,拍摄x线,CT,按Atlas对胸腰椎爆裂骨折的分类划分骨折类型并统计。测试正常标本及逐级撞击成功后各组标本三维运动范围。结果:终板预损伤组B型骨折9例,A型骨折1例;L1椎体预损伤组A型骨折5例,B型4例,C型1例。不做预处理组A型5例,B型3例,C型2例。终板损伤组B型骨折的数量明显多于L1椎体预损伤组、不做预处理组,且存在显著差异(P〈0.05)。L1椎体预损伤组和不做预处理组在逐级撞击产生爆裂骨折性类型上没有明显差别,统计分析没有显著差异(p0.05)。撞击成功后的各组标本前屈在后伸、左右弯、左右旋转的运动范围与正常标本相比均有明显增加,且存在显著差异(P〈0.01)。各组之间无明显差异(P〉0.05)。结论:终板预损伤逐级撞击法能够成功建立猪胸腰椎爆裂性骨折B型模型。上终板中后1/3的预损伤是B型模型成功的关键。Objective: To establish B-type experimental model of pig's thoracolumbar burst fi'acture. Methods: 30 pig fresh thoracolumbar specimens from T13-L3 were collected, and divided into three groups randomly: vertebral endplate pre-injury group, LI vertebra pre-injury group and no pre-injury group. Three dimensional motions of segment T14-T2 were measured on a spinal three motion test machine before and after trauma. Using drop weight technique and incremental trauma approach, burst fracture was produced. The specimens were examined by X-ray and CT. The results were caculated accroding to Atlas' burst fracture type. Regults: According to Atlas' type, 1, 9 and 0 cases in vertebral endplate pre-injury group, 5, 4 and 1 cases in L1 pre-injury group, 0, 3 and 2 cases in no pre-injury group belonged to A, B and C types respectively. The number of type B occurred in vertebral endplate pre-injury group significantly more than that in vertebrae pre-injury group and no pre-injury group (P〈0.05). No significant differences were found between pre-injury group and no pre-injury group (P〉0.05). The range of movement of the segments on flexion/extension, left/right bending and left/right rotation were significant increased in final trauma specimens compared to that of normal specimens (P〈0.01), however, no significant differences of ROM were found among three trauma groups. ConclusioN: It's an effective way to produce experimental model of type B thoracolumbar burst fracture using vertebral endplate pre-injury and incremental trauma approach. The pre-injury of endplate at the middle-posterior one-third of vertebral endplate is the key to make a type B thoracolumbar burst fracture model.
