摘  要:目的观察尿道括约肌各组织成分年龄变化,探讨与增龄相关的括约肌功能障碍的形态学基础。方法分别取6、12、24、32月龄雌性未生育Fisher大鼠28只,每组7只,尿道标本常规石蜡包埋切片,Masson’s Trichrome染色,光学显微镜观察并用计算机图像分析系统定量分析尿道括约肌各组织成分和空间形态的年龄变化。结果尿道横纹肌含量从12月龄开始减少;从24月龄开始尿道黏膜变薄、黏膜下血管数量减少、尿道嵴面积缩小、尿道腔最长径/最短径比值增大,胶原结缔组织含量增加;尿道背侧壁厚度变薄,尿道腔面积扩大仅见于32月龄组。尿道平滑肌和尿道总面积随年龄增加没有发生明显变化。结论随着年龄增长,雌性大鼠尿道横纹括约肌减少致关闭尿道的动力减弱,尿道黏膜变薄使密封效果减弱,尿道腔扩大使关闭距离增加等系列变化可能是增龄相关的尿道括约肌功能障碍性疾病的形态学基础。Objective To investigate the aging changes of histological components of the urinary sphincter in female rats and explore the morphology to discuss the mechanism of age-related urinary continence dysfunction. Methods Twenty-eight adult female nonporous Fisher rats at different ages were divided equally into 4 groups, group Ⅰ (6 months), group Ⅱ (12 months), group Ⅲ (24 months) and group Ⅳ (32 months). The dorsal urethra wall thickness, urethral lumen area, cross section area of crista urethralis, mucosa thickness and area, number of submucosal vessels, the mount of collagen and striated and smooth muscle fibers of the mid-urethra cross-sections of adult female rats at different ages were evaluated by computerized morphometric analysis system after staining with Masson' s Trichrome procedure. Results The amount of striated muscle fibers was decreased remarkably from 12 months old female rats. The mucosal area, the number of submucosal vessels and the cross section area of crista urethralis were decreased from 24 months ( each P 〈 0.05). While the amount of collagen (P = 0. 047 ) and the radio of the longest and shortest urethral diameters ( P = 0.049) were increased. The urethral lumen area (P = 0. 026) and the dorsal urethra wall thickness (P = 0. 034) were changed in 32 months rats. It was verified that the whole urethra area and the amount of smooth muscle fibers did not change significantly with aging ( each P 〉 0.05 ). Conclusion There are a series of changes in the microarchitecture and morphology of the urinary sphincter with aging. These changes provide insight into the understanding of the aging changes of urethral sphincter function.
