伤椎置钉技术在治疗胸腰椎骨折中的即时复位作用

摘　　要：目的探讨伤椎置钉技术治疗新鲜胸腰椎骨折的复位效果。方法对41例Magerl-Harms—Gertzbein分类为A、B1、B2型的胸腰椎骨折患者，采刚后路手术，在伤椎椎弓一侧或两侧置入长度较短的螺钉，在复位时通过直接推顶伤椎椎弓根钉，使得向后移位的椎体向前复位，恢复解剖序列。结果36例依靠单纯伤椎置钉技术取得骨折块的满意复位，5例在术中需用其他器械辅助进行椎管内骨片的复位。结论伤椎置钉技术对某些类型的新鲜胸腰椎骨折具有良好的即时复位效果。Objective To investigate the efficacy of reduction by fixing pedicle screws into the injured vertebra in treatment of thoracolumbar fractures. Methods A total of 41 patients with types A, BI and B2 （according to Magerl-Harms-Gertzbein classification） thoracolumbar fractures were treated surgically by using a posterior short segment pedicle screw fixation technique, with insertion of shorter screws into the fractured veterbra to reinforce the reduction and fixation. Results The height of the fractured vertebrae and the Cobb angle of the injured segment restored satisfactorily in 36 patients in this study. Five patients needed other technique to help the reduction of the bone graft extruding into the canal. Conclusion The procedure of pcdicle screw implantation into the injured veterbra has effective instant reduction of the fracture and dislocation in the treatment of some types of fresh thoracolumbar fractures.
