高血压患者左心房结构和功能的研究

摘　　要：目的：探讨高血压对左心房结构和功能的影响。方法：根据JNC7高血压诊断标准及左室质量指数，将患者分为高血压左室肥厚组、高血压非左室肥厚组。对照组为同期健康体检者。采用超声心动图测定左房容积及相关功能指标。结果：高血压非左室肥厚组、左室肥厚组与对照组间LVMI、Tei、E／A、LAD、LAVmax、LARV均有显著性差异（P〈0．01—0．05）；左室肥厚组和非左室肥厚组间LVMI、Tei、LAVmin有显著性差异（P〈0．05）。结论：左房重构发生在左室肥厚之前。随着高血压病情进展，左房功能渐失代偿。左房做功增加和结构改变均可反映左室舒张功能的异常。Objective： To explore the effects of essential hypertension on left atrial structure and function. Methods ： According to JNC7 diagnostic criteria of hypertension and left ventricular mass index, the subjects were divided into three groups：essential hypertension group（ n = 38 ） without left ventricular hypertrophy, essential hypertension group （n = 32） with left ventricular hypertrophy and healthy control group （n = 30）. The indexes that were related with left atrial structure and function were measured by echocardiography. Results： LVMI,Tei, LAD, LAVmax and LARV in the control group were significantly lower than those in other two groups （ P 〈 0.01 - 0.05 ）. LVMI, Tei and LAVmin in patients with left ventrieular hypertrophy were significantly higher than those without left ventricular hypertrophy （ P 〈 0.05 ）. Conclusion ： Left atrial reconstruction may occur before left ventricular hypertrophy. Along with the development of hypertension,left atrial function becomes decompensation gradually. Working elevation and struetural changes of left atrial may indicate the abnormality of its diastolic function in the earlier stage.
