摘 要:目的探讨门冬氨酸钾镁(PMA)在弥漫性轴突损伤(DAI)中的保护作用。方法选择符合DAI诊断标准的患者46例,随机分为常规组和PMA组,PMA组在常规组给予吸氧、脱水、器官功能支持、营养神经及预防并发症等综合治疗的基础上加用PMA治疗。于入院时、治疗后3 d、7 d、14 d分别检测血清NSE和MBP的浓度变化,并于入院时及入院1周和2周进行格拉斯哥昏迷评分(GCS)、3个月后进行格拉斯哥预后分级评分(GOS),以观察远期治疗效果。以同期20例健康体检者作为对照组。结果DAI患者血清NES和MBP的浓度均显著高于健康人(P<0.05)。治疗后7 d、14 d PMA组血清NSE和MBP浓度分别均低于常规组,差异均有统计学意义(P<0.05)。PMA组在入院时GCS与常规组比较,差异无统计学意义(P>0.05),但入院1周和2周的GCS及治疗3个月后的GOS均显著高于常规组,差异有统计学意义(P<0.05)。结论PMA能降低DAI患者血清NSE、MBP含量,并能明显改善患者的预后。Objective To study the role of potassium-magnesium aspartate in the diffuse axonal injury. Methods 46 patients with diffuse axonal injury were randomly divided into two groups. The treated group was treated with potassium-magnesium aspartate plus conventional treatments including oxygen, dehydration, organ functional support, nerve nutrition, prevention of complication. The conventional group was treated with conventional treatments alone. The concentrations of NSE and MBP in serum at admission and at 3,7and 14 days after treatment were determined; the Glasgow coma score (GCS) at admission and at 1 week and 2 weeks after hospitalization and the Glasgow outcome scale (GOS) after 3 months were compared to observe the long-term efficacy. Meanwhile, 20 healthy people,including 12 males and 8 females aged 32 -51 years old, were selected as control group. Results The concentrations of the serum NSE and MBP in the patients with diffuse axonal injury were significantly higher than those in the healthy adults (P 〈 0. 05). The concentrations of serum NSE and MBP in the treated group were significantly lower than those in the conventional group after the treated 7 and 14 days(P 〈 0.05). Before treatment, the GCSs of patients in the two groups had not statistical difference (P 〉 0.05). The GCSs of the patients at 1 week, 2 weeks and GOS at 3 months after treatment in the potassium-magnesium aspartate group were significantly higher than those in the conventional group (P 〈 0.05 ). Conclusion Potassium-magnesium aspartate can reduce the concentrations of serum NSE and MBP and improve the prognosis in the patients with diffuse axonal injury.
