摘 要:目的评价VITEK2分析系统的药敏结果的准确性,以判断其药敏结果能否满足临床需求。方法用VITEK2试卡法和K-B法同时检测51株肠杆菌科菌、24株鲍曼不动杆菌、27株铜绿假单胞菌以及20株葡萄球菌,并将VITEK2试卡法检测结果与参考方法K-B法进行对照分析。结果通过对1396株细菌-抗生素组合的检测,试卡法与K-B法的标准符合率(CA)为91.47%,严重错误(VME)为2.04%,重大错误(ME)为3.47%,一般错误(MIE)为5.95%。结论VITEK2全自动微生物分析系统的药敏结果具有较高的准确性,与K-B法有较好的关联,能满足临床的要求。该系统具有受人为因素的影响较小、操作简便、快速、重复性高的特性,可作为临床实验室常规药敏检测的选择方法。在进行甲氧苄啶/磺胺甲异噁唑的敏感性检测时,试卡法更为可靠。在进行部分药物(亚胺培南、哌拉西林/他唑巴坦以及苯唑西林)的敏感性检测时建议同时加做K-B法检测。Objective To evaluate the reliability of VITEK2 for antimicrobial susceptibility testing of clinical isolates. Methods One hundred twenty-two clinical isolates including 51 strains Enterobacteriaceae, 27 strains Pseudomonas aerations, 24 strains Acinetobacter baumannii and 20 strains Staphylococcus were tested by VITEK2 instrument and the results were compared with the results tested by the references method,K-B method. Results According to the breakpoints of CLSI,for 1396 microorganism-antibiotic combinations,VITEK2 gave 91.47% strains with category agreement (CA), 2.04% strains with very major errors (VME), 3.47 % strains with major errors (ME), 5.95 % strainswith minor errors(MIE). The VME rates in Enterobacteriaceae-Trimethoprim / Sulfamethoxazole,Pseudomonas aeruginosa-Piperacillin/tazobactam,Staphylococcus Oxacillin,Staphylococcus-Trimethoprim/Sulfamethoxazole was 8. 70% ,28.57% ,17. 65%,14.29% respectively. Conclusion The results from VITEK2 have a good correlation with the results of K-B method. Antimicrobial susceptibility test by VITEK2 instrument is rapid, simple, reproducible and suited to be a routine method for clinical microbiology laboratory.
