摘  要:目的探讨山莨菪碱预处理对兔肺缺血再灌注损伤的保护作用及其作用机制。方法建立兔肺缺血再灌注动物模型,健康家兔24只,随机分为3组,每组8只。假手术组不行缺血再灌注处理,对照组进行左肺缺血再灌注处理,山莨菪碱组静脉给予山莨菪碱预处理后行左肺缺血再灌注处理。各组不同时间点采血,测血浆丙二醛(MDA)和一氧化氮(NO)含量;术后取左肺组织进行形态学观察和还原型谷胱甘肽过氧化物酶(GSHpx)含量测定;最后通过静脉注射伊文思兰的方法比较3组肺血管通透性差异。结果缺血再灌注处理后,对照组血浆MDA和NO含量明显高于假手术组(P均<0.01),GSHpx含量明显低于假手术组(P<0.01),组织病理损伤严重,肺血管通透性高,山莨菪碱组上述指标水平改善(P均<0.05),组织损伤减轻,肺血管通透性低。结论应用山莨菪碱预处理可以显著减轻肺缺血再灌注损伤的程度。Objective To study the protective effect of Anisodamine on lung ischemia-reperfusion injury in rabbits,and to explore the possible mechanism. Methods Rabbits lung warm ischemia-reperfusion injury model was developed. Twenty-four Chinese rabbits were divided into 3 groups randomly:Sham group, Control group and Anisodamine group, 8 rabbits were in each group. In Sham group,left lung received no ischemia reperfusion. In Control group,the left pulmonary hilus was occluded for 1 hour and then released for 3 hours, did ischemia reperfusion. In Anisodamine group the left pulmonary hilus was occluded for 1 hour and then released for 3 hours, before occlusion, Anisodamine were given by intravenous injection. Each group collected venous blood at different time, plasma malondialdehyde (MDA) and nitrogen monoxidum(NO) level were checked. After operation,left lung tissue was harvested for histopathology analyses and glutathione peroxidase(GSHpx) content checked. At last 20/00 Evens blue (1.5mL/kg) was injected into femoral venous in order to measure lung vascular permeability by the method of Udaka. Results After reperfused, plasma MDA and NO content in Control group raised apparently and was significantly more than that in Sham group (P〈0.01), the lung tissue GSHpx level in Control group was significantly lower than that in Sham (P〈0.01) ,left lung revealed prominent edema, septal swelling, hemorrhage and neutrophils infiltration,lung vascular permeability obviously higher than Sham group (P〈0.01 ). These items in Anisodamine group were significantly improved compare with Control(P〈0.05). Conclusion Anisodamine can significantly prevent lung from ischemia-reperfusion injury. The mechanism may be inhibiting lipid peroxidation and neutrophils infiltration,depressing vascular permeability and pneumonedema.
