联合微创手术治疗单纯大隐静脉曲张316例疗效评价

摘　　要：目的：探讨高位结扎、顺行内翻式全程剥脱联合电凝术在大隐静脉曲张中的治疗效果。方法：所有病例均采用大隐静脉主干高位结扎、顺行内翻式全程剥脱联合曲张属支静脉电凝术（部分严重团状静脉曲张联合点式剥脱术）治疗,术后给予抗血小板治疗半个月,循环驱动器辅助3d及配穿循序减压弹力袜3个月。结果：全组患者术中、术后均顺利,所有患者未发生深静脉血栓形成。随访3个月-3年,仅1例术后1年于CockettⅡ交通支位置出现局部膨起,再次局麻下行交通支结扎并缝合该处深筋膜缺口后痊愈,其余病例无复发,且患肢不显瘢痕,达到美容效果。结论：高位结扎、顺行内翻式全程剥脱联合电凝术可适用于各种程度的大隐静脉曲张,且治疗效果明显优于传统手术,费用较低,便于在基层推广应用。Objective： To evaluate the therapeutic effect of applying high ligation and anterograde varus striping operation with concomitant electrocoagulation treatment of simple varicosis of great saphenous vein. Methods： All the cases in this group were carried out high ligation and anterograde striping operation with concomitant communicating branches electrocoagulation treatment （some severe agglomerate varicosis underwent punctiform stripping operation）. After operation, every case were given antiplatelet treatment for half a month, assisted with circulation driver for three days and pulled on decompression elasticity socks for three months. Resuits： All the cases in this group had healing by first intention without infection. There were no deep venous thrombosis. After 3 months- 3 years＇ followed up, there were no recurrence and less scars except one case. Conclusion：The high ligation and anterograde varus striping operation with concomitant electrocoagulation treatment can be underwent for different grade great saphenous vein varicosity and more effective than the traditional operation. The procedure is highly recommended.
