摘 要:目的应用彩色多普勒感兴趣区定量技术评价糖尿病患者早期肾脏末梢血流灌注,探讨其临床意义。方法以正常对照组(30例)、单纯糖尿病组(DM组,20例)及糖尿病合并高血压组(DM+EH组,16例)为研究对象,检测叶间及小叶间动脉收缩期峰值流速(PSV)、舒张期末流速(EDV)和阻力指数(RI),应用彩色多普勒感兴趣区定量技术对右肾包膜下皮质区血流进行分析,获得血管指数(VI)、血流指数(FI)和血管血流指数(VFI),3组间进行比较。结果DM组小叶间动脉、DM+EH组叶间动脉及小叶间动脉EDV均低于对照组,RI均高于对照组,差异均有统计学意义(P<0.05或P<0.01);DM组及DM+EH组VI、VFI均低于对照组,两者的达峰时间大于对照组,差异均有统计学意义(P<0.01或P<0.05);DM组及DM+EH组FI与对照组比较差异无显著性意义(均P>0.05),而其达峰时间较对照组延长(P<0.05或P<0.01)。结论糖尿病患者在出现实验室检查肾功能异常之前,已出现肾脏末梢血管床减少,血流灌注量减低及血流灌注时间延长。彩色多普勒感兴趣区定量技术能够准确评价糖尿病早期肾脏末梢血流灌注。Objective To evaluate renal peripheral inflow perfusion in patients with diabetes mellitus(DM) by color Doppler technique of region of interest(ROI) quantification.Methods Thirty healthy controls,20 cases of DM and 16 cases of DM complicated with essential hypertension(EH) were measured for renal peak systolic velocity(PSV),end diastolic velocity(EDV) and restrictive index(RI).The blood flow of cortical area under the capsule in kidney was analyzed by color Doppler technique of ROI quantification to obtain vascularization index(VI),flow index(FI),vascularization flow index(VFI).Results EDV of intralobular artery in DM and DM+EH groups and of interlobar artery in DM+EH group was lower than controls,while RI of intralobular artery in DM and DM+EH groups and of interlobar artery in DM+EH group higher than controls(P〈0.05 or P〈0.01).VI and VFI in DM and DM+EH groups were lower than controls(P〈0.05 or P〈0.01).Time to peak VI,time to peak VFI,and time to peak FI in DM and DM+EH groups were longer than in controls(P〈0.05 or P〈0.01).However,FI in DM and DM+EH groups had no significant difference with that of controls(P〉0.05).ConclusionRenal peripheral inflow perfusion in DM patients changed before renal dysfunction by laboratory examination appeared.Vascular beds of kidney in DM patients were decreased,inflow perfusion reduced,while time of inflow perfusion prolonged.Color Doppler technique of ROI quantification can accurately evaluate renal peripheral inflow perfusion in DM patients.
