摘  要:目的探讨胰激肽原酶(pancreatic kzllidinogenase,PK)减少体外冲击波碎石(ESWL)时对肾功能的损伤。方法40例肾盂结石进行ESWL治疗,随机分为PK组及对照组,各20例。PK组ESWL前3d开始肌注PK。采用放射免疫法测定ESWL前后尿β2-微球蛋白(β2-MG)、白蛋白(Alb)的变化。结果ESWL后对照组尿β2-MG、Alb明显高于ESWL前水平(P〈0.01);PK组尿中Alb比ESWL前轻度增加(P〉0.05),但与对照组比较差异有统计学意义(P〈0.01),而PK组尿中β2-MG较ESWL前明显增加(P〈0.01),组间比较差异无统计学意义(P〉0.05)。结论PK可以减少尿微量蛋白的排泄,对ESWL时肾功能损害有保护作用。Objective To investigate whether the pancreatic kininogenase (PK) can protect the renal function from damage due to ESWL. Methods The patients were randomly divided into the PK group and the control group. PK was given to the PK group for 3 days before ESWL but not to the control group. β2- MG and Alb in 24 - h urine before and after ESWL were measured by radio - immunological assay. Results In the control group, urinary β2- MG and Alb after ESWL were significantly increased as compared with those before ESWL ( P 〈 0.01 ). In the PK group, urinary Alb was slightly increased after ESWL as compared with that before ESWL ( P 〉 0.05 ), and there was significant difference between PK and control groups ( P 〈 0.01 ) ; urinary β2- MG was significant increased after ESWL as compared with that before ESWL ( P 〈 0.01 ), but there was no significant difference between two groups (P 〉 0.05). Conclusion PK can reduce the excretion of urinary protein and would protect the renal function form damage due to ESWL.
