外伤性结肠破裂79例手术治疗分析

摘　　要：目的探讨创伤性结肠损伤的处理方法以及一期手术的安全性和有效性。方法外伤性结肠破裂患者86例，79例行急诊手术治疗，其中行单纯修补手术37例，结肠部分切除Ⅰ期吻合术29例，结肠造口13例。结果本组病例合并多发伤患者50例，有5例患者因胸腹部严重复合伤、术后肠瘘、失血性休克等原因死亡，其余患者均痊愈出院。结论外伤性结肠损伤一期手术对绝大多数结肠损伤是安全可靠的，值得临床应用。Objective To explore the treatments for traumatic colonic rupture and evaluate the safety and efficacy of primary operation. Methods Of 86 patients with traumatic colonic rupture from May 2003 to February 2008,79 cases received emergency surgery,including 37 cases receiving primary repair,29 cases primary anastomosis after excision,and 13 cases colostomy. Results Among the 79 cases,50 cases were complicated with multiple organ injuries. Five patients died from serious combined injury,intestinal fistula after operation and hemorrhagic shock etc. Seventy - four cases were cured. Conclusion Primary operation of traumatic colonic injury is safe and reliable for most patients with colonic injury.
