摘　　要：目的通过收集并分析昆明市社区卫生服务机构工作和资源配置状况,评价其所处建设水平,探索对"六位一体"服务的评估方法,为社区卫生服务的发展和建设提供参考。方法以2005年昆明五区八县一地市社区卫生服务机构年报作为信息来源,统计并描述卫生及人力资源配置、培训、管理、"六位一体"服务等情况,以TOPSIS法对各区县"六位一体"服务综合评价。结果有92.03%(150/163)的合格年报进入统计分析,各机构平均服务社区居民(21914±24954)人。平均业务用房(867.26±1351.84)m2,有53.0%(79/150)是定点医疗机构,40.9%(61/150)与医保联网。150个社区卫生机构有从业人员2358人,医、护、防保人员比为4.48:3.72:1.00。每千人口有社区卫生人员0.72人。主要医疗设备配置合格率为54.31%。76.3%的机构有规章、考核制度和工作月报。"六位一体"服务综合评价从优到劣依次为五华区、官渡区、盘龙区、东川区、九县(市)区、西山区。结论2005年昆明城市社区卫生服务体系框架基本建立,但其基础资源配置、培训、管理、"六位一体"服务等均有待加强。基于等权的TOPSIS法可用于全面评价并比较机构的工作情况。Objective Through a survey of urban community health services and human health resources in Kunming City based on the allocation of resources, training and management to evaluate their level positions, to explore the evaluation method of ＂Six in One＂ service, and to provide a reference for the future development and construction. Methods We used the annual reports from the five reg ons and eight counties＇ community health agencies of Kunming City in 2005, as the source information, to count and describe the health and human resources, training, management, ＂Six in One＂ services, etc. ＂Six in One＂ services were comprehensively evaluated by TOPSIS method Results There were 1.50 qualified annual reports （92.03 % ） to enter the statistical analysis. Each agency served an average of 21,914 ± 24,954 community residents, and owned an average of 867.26 ± 1,351.84 m^2 operational area. Among the 150 community health agencies, 53.0 % （79/ 150） of them were fixed- point medical institutions, 40.9 % （61/150） were networked with the medical insurances. There were 2,358 community health workers in the 1.50 community health agencies; the ratio of doctor, nurse, prevention and health care personnel was 4.48 ： 3.72 ： 1.00. Community health worker per one thousand residents was 0.72 person. The pass rate of major medical equipment configuration was .54.31%. 76.3% of the community health service agencies had regulations, checkup system, and monthly working report. Comprehensive evaluation of ＂Six in One＂ services in community health services showed that Wuhua District had the best services, followed by Guandu District, Panlong District, Dongchuan District, nine counties （cities/areas）, and Xishan District. Conclusions The basic framework of the urban community health service system is established in Kunming in 2005, but the allocation of basic resources, training, management, and ＂Six in One＂ service need to be strengthened.
