乳腺浸润性导管癌超声征象与病理组织学分级及ER、PR表达的相关性研究

摘　　要：目的探讨乳腺浸润性导管癌超声征象与病理组织学分级及ER、PR表达的相关性。方法44例乳腺癌均经病理组织学证实,且所有病例在治疗前均行乳腺超声检查,术后进行病理组织学分级及测定ER、PR的表达。分析乳腺癌的超声表现与病理组织学分级及ER、PR表达的相关性。结果浸润性导管癌的组织学分级与肿块大小二者之间呈正相关变化趋势;超声征象与病理组织学分级之间无明显相关性;组织学分级与血流分级呈正相关变化趋势。毛刺征组、周边强回声晕组、后方回声衰减组ER、PR表达的阳性率较无毛刺组、无周边强回声晕组、无后方回声衰减组高;ER、PR的表达与血流丰富程度无明显相关性;随着肿块体积的增大,ER、PR表达的阳性率越低。结论乳腺浸润性导管癌超声征象与病理组织学分级及ER、PR的表达密切相关,对病理组织学分级有提示作用,且在一定程度上反映了ER、PR的表达状态。Objective To investigate the relationship between breast invasive ductal carcinoma ultrasonographic features and the pathologic histology grading, the expression of ER and PR in breast invasive ductal carcinoma. Methods Forty - four patients with breast invasive ductal carcinoma confirmed by pathology were examined with ultrasonography before operation. After operation, the pathologic histology grading was performed and the expression of ER and PR was determined. The relationship between ultrasonographic features with the pathologic histology grading and the expression of ER and PR were analyzed. Results A positive correlation was found among the pathologic histology grading of invasive ductal carcinoma; tumor size and flow grading. But there was no significant correlation among speculated margin, echogenic halo, posterior shadowing, microcalcification and the pathologic histology grading. The expression of ER and PR had a relative higher level in the tumor with the burr sign, high- level echo acoustic hale and posterior echo attenuation. The positive expression rates of ER and PR in patients with tumor diameter 〉2 cm were lower than those in patients with tumor diameter 〈2 cm. Conclusions The ultrasonographic features of breast invasive ductal carcinoma are closely correlated with the pathologic histology grading and the expression of ER and PR. It can be a predictor of histology grading and can reflect the expression state of ER and PR.
