不同皮瓣修复手指末端皮肤软组织缺损282例分析

摘　　要：目的：探讨手指末端皮肤软组织缺损功能性修复的几种方法。方法：采用九种皮瓣,根据指端缺损创面进行修复,共265人、282指：用指掌侧V-Y推进瓣修复43例,指神经血管束V-Y岛状推进瓣修复27例,指掌侧带指神经血管束推进瓣修复33例,示指背侧神经血管蒂岛状瓣修复拇指17例,指总神经血管束岛状瓣修复12例,带神经拇指尺背侧动脉逆行岛状瓣修复拇指25例,带神经指动脉背侧支逆行岛状瓣修复85例,带指神经指动脉逆行岛状瓣修复22例,带指神经背侧支指动脉逆行岛状瓣修复18例。结果：279例皮瓣存活,3例坏死,成活率98.9%,随访3～12个月,指端外形满意,感觉良好。结论：带神经皮瓣皮肤质地结构与患处相似,感觉功能恢复良好,是修复指端缺损的理想材料。Objective： To discuss several flaps in repairing of fingertip soft tissue defects.Methods： Nine types of skin flap were selected to repair fingertip soft tissue defects in 265 patients,282 fingers.Results： All flaps but three survived.A follow-up（3 to 12 months） was made in all patients,the fingertips had satisfactory appearances and sensations,and the functions of finger were excellent.Conclusion： The texture and architecture of hand flaps are similar to fingers,which are the ideal material for reconstruction of finger-tip soft tissue defects.
