原发性高血压患者血压负荷值与无症状性心肌缺血的相关关系的探讨

摘　　要：目的探讨原发性高血压患者血压负荷值与无症状性心肌缺血的关系。方法选择同时监测24 h动态血压(ABPM)及动态心电图(DCG)的高血压病患者120例和30例血压正常。根据血压负荷值:轻度负荷(10.1%～40.0%)、中度负荷(40.1%～80.0%)、重度负荷≥80.1%。根据24 h血压负荷把患者分轻度、中度、重度三组分别与正常组比较,中度、重度组与轻度组比较,重度组与中度组比较;在同等程度血压负荷下分白昼组和夜间组,中度、重度组与轻度组比较(白昼组间、夜间组间),重度组与中度组比较(白昼组间、夜间组间),在同等程度血压负荷下夜间组与白昼组进行比较,分别比较不同组间ST段压低程度、持续时间及发作次数。结果24 h血压负荷值各组比较:轻度负荷组与正常组比较有显著性差异(P<0.05),其他各组均有非常显著性差异(P<0.01);白昼、夜间血压负荷值各组比较:轻度负荷夜间与白昼比较无明显差异,其他各组均有非常显著性差异。结论高血压病患者无症状性心肌缺血的发作频率、持续时间随着24 h、白昼、夜间血压负荷值增高而增加,特别是中度负荷以上,且在同等程度负荷下夜间比白昼更明显。Objective To investigate the relationship between ambulatory blood pressure loading value and silent myocardial isehemia in patients with essential hypertension. Methods DCG and ABPM were perfomed in 120 patients with essential hypertension and 30 cases of normal blood pressure. According to the blood pressure loading value ： lightly loading（ 10. 1% - 40. 0% ）, moderate loading （40. 1% - 80. 0% ）, severe loading ≥80. 1%. The patients 24 h blood pressure loading value are divided into 3 teams：lightly,moderate and severe with normal group compare, to compare moderate, severe with lightly group, to compare severe with moderate. The load to the same erten are divided into day team and night team, to compare moderate, severe with lightly group（ during the day group, during the night group）, to compare severe with moderate （during the day group, during the night group）. The load to the same erten to compare night with day. Compare their deference group ST-segment depression（ duration,maximal ST segment depression and its incidence）. Results To compare 24 h blood pressure loading value deference group：To compare lightly with normal group is obviously different （P 〈 0. 05 ） ; Etcaeteras deference group are differ sharply（ P 〈 0. 01 ） ;To compare the day with night blood pressure loading value deference group：lightly loading night with day group has no obvious difference, Etcaeteras deference group are differ sharply. Conclusion 24 h, day and night silent myocardial ischemia was more frequen and lasted longer along with the blood pressure loading value increased. In particular, more than moderate load, the load to the same ertent the more obvious at night than day.
