摘　　要：目的介绍应用Ｂｕｎｎｅｌ带双金属针双钮扣肌腱缝合线治疗锤状指的疗效。方法对１２例新鲜锤状指、４例陈旧锤状指，用Ｂｕｎｎｅｌ带双金属针双钮扣肌腱缝合线（德国产）缝合肌腱断端，即利用缝合线本身张力固定远节指间关节（ＤＩＰ）于过伸１０～２０度位，断端用３－０无创尼龙针线行褥式缝合，术后不用外固定。结果１例因感染于术后８周再度断裂，１例于术后１２周因再次外伤缝合肌腱断裂而导致失败；其它１４例的ＤＩＰ关节背伸达０～５度，屈曲６０～８０度，关节屈伸活动度平均在７６．３度左右，无１例发生ＤＩＰ关节僵硬和功能障碍。结论采用Ｂｕｎｎｅｌ带双金属针双钮扣肌腱缝合线治疗锤状指，操作简单、疗效可靠。Objective To introduce and evaluate treatment outcome of mallet fingers using double metal needle and double button Bunnell's tendon suture. Methods In 12 fresh and 4 old mallet fingers, the broken tendons were sutured with double metal needle and double button Bunnell's suture (Made in Germany). Matress suture using 3 0 nylon suture were added. The suture tension maintained the DIP joints in extension of 10～20 degrees. No external fixator was used. Results The DIP joints of 14 cases could extend to 0～5 degrees and flex to 60～80 degrees, with an average motion range of 76.3 degrees. Two cases failed because of tendon rupture due to infection at 8th week in one case and trauma at the 12th week in another. No stiffness or dysfunction of the DIP joints was found. Conclusions It is easy and effective to treat mallet fingers using double metal needle and double button Bunnell's tendon suture.
