摘 要:[目的]分析重症监护病房(ICU)下呼吸道病原菌的耐药状况,总结病原菌的感染特点,为临床诊治提供参考。[方法]对2006年1月~2007年12月成都中医药大学附属医院重症监护病房下呼吸道送检标本中分离的致病菌株及其药敏试验结果进行回顾性分析。[结果]在307株病原菌中G-杆菌211株(68.60%),G+球菌57株(18.50%),真菌39株(12.69%)。药敏结果显示,多数G-杆菌耐药性呈逐年升高趋势,且呈多重耐药,对β-内酰胺类抗生素和喹诺酮类的耐药性上升尤为显著,铜绿假单胞菌和鲍曼不动杆菌对亚胺培南耐药性也在逐年上升,且有泛耐菌株出现。葡萄球菌除万古霉素显敏感和复方新诺明耐药率低于70%外,对其余常用抗生素均具有较高的耐药性。[结论]上述结果有助于了解重症监护患者继发感染细菌的流行病学特征与其耐药性,有利于指导临床合理使用抗菌药物。[Objective] To investigate the antibiotic-resistanee and eharaeteristies of respiratory pathogenic bacteria in ICUs, and to offer reasonable evidence to clinical therapy coneenred. [ Methods] The clinical characteristics of isolated pathogenie strains and the results of drug sensitivity tests in patients with respiratory disease who were treated in our hospital were reviewed and analyzed. [ Results] From the collected 307strains. the Gram-negative baeilli (211 strains, 68.60%) were the dominant majorities. Grarn-positive cocci (57 strains) and fungi (39 strains) accounted for 18.50% and 12.69%, respectively. From the results of drug sensitivity tests, most Gram-negative bacilli were becoming more and more resistant to antibi- otics, especially Beta-lactam antibioties and Quinolones. Pseudomonas aeruginosa as well as acinetobaeterbaumanii were becoming more and more resistant to Impenem. Additionally. Pan-resistant strains were. isolated. Staphylococcus exhibited high resistance to common antibiotics, while sensitive to Vancomycin, and tire resistant rate to Compound Sulfamethoxazole were below 70%. [ Conclusion ] These findings contribute to understanding the epidemiologieal characteristics and the antibiotic-resistance of the intensive care panents with secondary bacterial infection, arid guiding rational use of antibiotics.
