干燥综合征40例临床特点分析

摘　　要：目的探讨原发性干燥综合征(pSS)和类风湿关节炎继发干燥综合征(RA/SS)的临床表现、辅助检查及实验室检查的特点,提高对干燥综合征(SS)的诊断认识。方法回顾对比分析40例干燥综合征,其中13例原发性干燥综合征,27例类风湿关节炎继发干燥综合征的临床资料,包括口干、眼干、猖獗龋齿、关节/肌肉痛、关节肿胀、晨僵等临床表现,唾液流率、Schirmer试验、唇腺活检、角膜荧光染色、双手X线辅助检查,及抗核抗体(ANA)、抗可溶性酸性核蛋白SSA/SSB抗体(抗SSA/SSB)、类风湿因子(RF)、肝功实验室检查。结果在关节/肌肉痛、关节肿胀、晨僵、RF及双手X线骨侵蚀方面,RA/SS阳性发生率高于pSS(P<0.05);唇腺活检、角膜荧光染色、抗SSA/SSB检查阳性发生率pSS高于RA/SS(P<0.05),而其他的临床表现、辅助检查及实验室检查pSS和RA/SS阳性发生率无差异(P>0.05)。结论关节/肌肉痛、关节肿胀、晨僵、双手X线骨侵蚀、唇腺活检、角膜荧光染色、抗SSA/SSB对pSS与RA/SS的鉴别诊断有临床指导意义。Objective To improve the diagnosis cognition of Sjogren＇ s syndrome（SS） by analyzing the clinical characteristics of primary Sjogren＇ s syndrome（pSS）and secondary SS complicated with rheumatoid arthritis（RA/SS）. Methods The characteristics of 40 patients with SS（ 13 cases of pSS and 27 cases of RA with SS）were analyzed, respectively. The clinical data including dry mouth, dry eyes, inflame joints, morning stiffness and accessory examinations of flow of saliva, lip biopsy, fluorescent stain in cornea, X-ray results of two-hand, and laboratory examinations of antinuclear antibody （ANA）, anti-SSA/SSB, and rheumatoid factor（ RF）, were studied. Resuits The incidence rates of inflame joints, morning stiffness,RF,bone erosion of two-hand by X-ray in RA/SS patients were significantly higher than that in pSS patients （P 〈 0.05 ）. The positive rates of lip biopsy, fluorescent stain in cornea, anti-SSA/SSB in pSS patients were higher than that in RA/SS patients （ P 〈 0. 05 ）. Conclusion These indices and tests, such as, inflame joints, morning stiffness, lip biopsy, fluorescent coloration in cornea, anti-SSA/SSB, and X-ray of two-hand may be useful for the differential diagnosis of pSS and RA/SS.
