摘  要:目的:比较肱动脉袖带血压与中心动脉血压的差异性,并探讨两者间差异的可能影响因素及机制。方法:选择住院行诊断性冠脉造影患者128例,平均(60.87±9.36)岁。术前测量脉搏波传导速度(PWV),容量顺应性(C1),振荡顺应性(C2),术中记录升主动脉压力(中心动脉血压,CAP),并同步测量肱动脉袖带血压。结果:(1)根据中心动脉与肱动脉袖带收缩压(SBP)差异程度分为3组:相符组(两者相差≤4mmHg)21例(16.40%),高估组(后者高于前者,差值>4mmHg)14例(10.94%),低估组(后者低于前者,差值>4mm-Hg)93例(72.66%);(2)低估组人群传统心血管危险因素多,多为老年,高血压、冠心病较多(P<0.05);(3)低估组PWV明显增快(P<0.05);(4)低估组C2明显降低(P<0.05)。结论:中老年人肱动脉袖带血压大多数低于中心动脉压,中心动脉硬化可能与之有关。Objective: To analyze the differences between brachial artery pressure and central aortic pressure, and to investigate the factors and possible mechanisms of brachial blood pressure overestimate or underestimate central aortic pressure. Methods.. A total of 128 patients [aged (60. 87±9. 36)] were enrolled. Central aortic pressure was invasively determined by catheterization and brachial artery pressure was non-invasively determined by sphygmomanometer simultaneously. The volume compliance (C1) and shock compliance (C2) were detected by HDI CV Profilor DO- 2020 and pulse wave velocity (PWV) was detected by BP--203RPEII (VP--1000). Results: (1) The patients with SBP of brachial systolic blood pressure agree to central aortic pressure (difference≤4 mmHg) were 21 cases (16.40 %), brachial systolic blood pressure overestimates central aortic pressure SBP (difference〉4mmHg) were 14 cases (10. 94%), brachial systolic blood pressure underestimates central aortic pressure ( difference〉4 mmHg) were 93 cases (72.66%); (2) Patients in underestimated group had more traditional cardiovascular risk factors: elderly , hypertension and coronary heart disease, etc. (P〈0.05) ; (3) PWV was faster in underestimated group (P〈0. 05) ; (4) C2 was lower in underestimated group (P〈0. 05). Conclusion: (1) The majority patients are with braehial systolic blood pressure underestimates central aortic pressure in middle aged and aged patients; (2) Aortic arteriosclerosis are correlated with peripheral SBP underestimates central SBP.
