第5掌骨颈骨折伸直与屈曲位整复固定治疗的比较研究

摘　　要：目的:研究伸直掌指关节整复、纸夹板固定治疗第5掌骨颈骨折的优越性。方法:74例患者随机分为2组,伸直掌指关节整复固定组(A组)与屈曲掌指关节整复固定组(B组)各37例。经整复、固定后,定期复查,记录X线片及功能恢复情况,依据中国中医药行业骨伤专业标准评定疗效。结果:A组固定效果均强于B组(P<0.05),B组在刚拆固定时功能强于A组(P<0.0001),但A组在第8周功能恢复即开始强于B组(P<0.01),第24周功能评分2组比较,差异无统计学意义(P=0.4364),说明2组最终功能恢复程度差异小,A组功能恢复快于B组。结论:伸直掌指关节整复、纸夹板固定的方法,其固定效果、功能恢复优于屈曲掌指关节整复固定的方法。Objective：To study the advantage of metacarpophalangeal joint reduction at extension position, and fixation with paper splint for the treatment of fracture of the neck of the fifth metacarpal. Methods：Seventy--four patients were randomized to two groups, 37 patients in each. Group A was treated with metacarpophalangeal joint reduction at extension position plus external fixation with paper splint, while group B was treated with metacarpophalangeal joint reduction at flexion position and fixation. After reduction and fixation, the patients underwent periodical reviews, and their X-ray and functional recovery were recorded. The curative effect was assessed according to the traumatology and orthopaedic standard of traditional Chinese medicine industry of China. Results：The fixation effect of group A was better than that of group B （P〈0. 05） while the function of group B was better than that of group A upon removing the fixation （P〈0. 0001）. However, in the 8th week, function recovery of group A was better than that of group B （P〈0.01） ; in the 24th week, there was no remarkable difference between the 2 groups in function assessment （P= 0. 4364）, indicating that there was no significant difference between the two groups in function recovery and that function recovery of group A was faster than group B. Conclusion： The method of metacarpophalangeal joint reduction at extension position plus paper splint with pressure pad has more advantages over the flexion position, and paper splint fixation has relatively good fixation effect and security.
