增生型糖尿病视网膜病变玻璃体切割手术后并发症的临床研究

摘　　要：目的对增生型糖尿病视网膜病变(PDR)玻璃体切割手术后并发症进行分析研究。方法PDR患者54例62眼,行标准玻璃体手术,术后随访2-12个月,观察手术并发症的发生情况。结果在54例62眼PDR玻璃体切割手术并发症中,发生视网膜脱离8眼(12.9%),玻璃体再出血24眼(38.7%),并发性白内障46眼(74.2%),新生血管性青光眼7眼(11.3%),视网膜血管病变21眼(33.9%),视神经萎缩9眼(14.5%)。结论PDR手术并发症的发生主要与病变程度、手术技巧、术中术后是否正确彻底处理眼部每一个病灶有关。术中、术后正确处理眼部病变,同时术前、术后兼顾全身情况,可不同程度避免手术并发症的发生。Objective To analyze the complications of vitrectomy in treatment of proliferative diabetic retinopathy（PDR）.Methods 62 eyes of 54 cases with PDR treated with vitrectomy were followed up 2-12 months,and the postoperative complications of vitrectomy were observed.Results Among the complications of these cases,retinal dethchment in 8 eyes（12.9%）,postoperative vitreous hemomhage in 24 eyes（38.7%）,complicated cataract in 46 eyes（74.2%）,neovascular glaucoma in 7 eyes（11.3%）,retinal vessel atresia in 21 eyes（33.9%）, optic nerve atrophy in 9 eyes （ 14.5% ）. Conclusion The main influential factor are related with the severity of PDR, surgical skills and methods for dealing with every pathological area correctly and thoroughly. Systemic status is also important factor to reduce the complications.
