无气腹腹腔镜在妇科手术中的临床应用

摘　　要：目的:探讨无气腹腹腔镜手术在妇科的临床应用.方法:对2008年6月～2009年6月我院妇科施行的无气腹腹腔镜手术100例临床资料进行回顾性分析,与同期开展的100例气腹腹腔镜手术在手术适应证、并发症、麻醉安全性、操作方法、经济费用等方面进行对比.结果:100例无气腹腹腔镜手术均顺利完成,硬膜外或腰硬联合麻醉满意,无明显并发症发生,术中出血量二者基本相近,手术时间明显缩短,其中附件手术、子宫肌瘤剔除术、腹腔镜辅助阴式子宫切除术分别减少49.3min、61.1min、14.0min.结论:无气腹腹腔镜手术在妇科的临床应用具有适应证广、安全省时、简单易行、经济实用的明显优势,非常适合在人口众多的发展中国家开展.Objective： To evaluate the clinical application of laparoscopic surgery without pneumoperitoneum in gynecology. Methods： From June 2008 to June 2009,clinical data of 100 cases which underwent gynecological laparoscopic surgery without pneumoperitoneum were retrospectively analyzed,compared the data with the 100 cases of pneumoperitoneum laparoscopic surgery which carried out with the same time in the aspects of surgical indications,complications,anesthesia safety,operative method,economic cost. Results： 100 cases of laparoslopic surgery without pneumperitoneum were successfully completed,epidural or combined spinalepidural anesthesia was satisfied,no significant complications,bleeding volume were similar in the two methods,operative time was significantly shorter.The time of annex surgery,uterine myomectomy,laparoscopic assisted vaginal hysterectomy decreased 49.3min,61.1min,14.0min.Conclusion： Laparoscopie surgery without penumopelitoneum has obvious advantages in gynecological clinical application such as wide indication, safety and time saving,economical and practical,simple and easy to do,so,it is very suitable for the developing countries with a large population.
