盐酸戊乙奎醚用于小儿全麻术前用药的临床观察

摘　　要：目的对盐酸戊乙奎醚和阿托品作为小儿全麻术前用药进行随机双盲比较。方法126例小儿全麻患者分为三组,术前静脉注射盐酸戊乙奎醚注射液、硫酸阿托品注射液或注射用水,测定注射后30min唾液分泌量,用视觉模拟评分方法测定口干程度,记录不良事件。结果给药后30min,盐酸戊乙奎醚、硫酸阿托品、注射用水组唾液量分别较用药前变化(-14.64±19.63)mg、(-22.50±24.04)mg、(2.74±19.45)mg。其中,盐酸戊乙奎醚和阿托品用药前后的下降有统计学意义(P<0.05),两组间无明显差异。VAS口干评分,盐酸戊乙奎醚、硫酸阿托品用药前后明显增加,两组间无明显差异;用药后30min,盐酸戊乙奎醚、硫酸阿托品两组用药前后的心率变化有统计学意义(P<0.05),观察期间无严重的不良反应。结论新型选择性抗胆碱药盐酸戊乙奎醚可明显减少唾液分泌,无心率增加,无明显不良反应,使用安全,是较理想的小儿全麻术前用药。Objective To compare the effects of penehyclidine hydrechloride and atropine used as premedication in children general anes- thesia. Methods We randomly allocated 126 children who would receive general anesthesia into 3 groups：injection of penehyclidine hy- drochloride, atropine or water as premedication, respectively, The secretion of salivary gland was measured and the thirst degree was determined by using visual analogue scale（VAS） and the adverse events were evaluated 30min after drug injection. Results The secretion of salivary gland was significantly decreased in penehyclidine hydrochloride and atropine groups（P 〈 0.05 ）, but 30min after drug injection, there was no differ- ence between the two group. The VAS was significantly increased in penehyclidine hydrochloride and atropine groups（P 〈 0.05 ） but 30min af- ter drug injection,there was no difference between the two groups. The heart rate was significantly decreased in penehyclidine hydrochloride group and was increased in atropine group, with a significant difference between the two groups 30min after drug injection （P〈 0.05）. No serious adverse events were reported. Conclusion Penehyclidine hydrochloride as premedication in children general anesthesia has the ad- vantages of decreased salivary gland secretion, no heart rate increase and no adverse events, and therefore, it is effective and safe.
