PPH治疗重度痔病152例探讨

摘　　要：目的探讨吻合器痔上黏膜环形切除术（procedure for prolapse and hemorrhoids，PPH）治疗重度痔病的价值和手术方法。方法回顾性分析2001年10月至2006年9月间采用PPH治疗重度痔病152例的临床资料。结果手术时间15-30min；术后3d出院。术后2个月随访，148例外脱痔块完全回缩，4例外脱痔块回缩不全；对手术效果表示满意140例，表示不够满意12例，包括手术后出血3例、外脱痔块回缩不全4例、外痔皮赘残留5例。全组无一例发生大便失禁、肛门狭窄或肛周脓肿等。结论采用两个半荷包缝合，必要时附加外痔皮赘切除，巨大痔块上部部分痔组织切除和痔上黏膜柱状缝合悬吊有利于提高手术效果。Objective To discuss the value and operating method of procedure for prolapse and hemorrhoids（PPH） in the treatment of severe hemorrhoids. Methods 152 cases of severe hemorrhoids subject to surgical treatment by PPH in our hospital from Oct. 2001 to Sept. 2006 were reviewed retrospectively. Results Operating time was 15 30 min. All patients discharged at the 3rd day after operation. Hemorrhoids in 148 cases disappeared after operation, and 140 cases were satisfaction with the treatment efficacy. After operation, there were 3 cases of bleeding,4 cases of hemorrhoids un-complete recovery,and 5 cases of external hemorrhoid. There was no postoperative eneopresis, anal stenosis or perianal abscess. Conclusion The two half purse string suture and PPH combined with external hemorrhoid incision or local rectal mucosa suture above the dentate line can improve the PPH efficacy.
