外翻张力固定预防和治疗小儿肱骨髁上骨折后肘内翻  

Valgus Tension Fixture in Prevention and Treatment of Cubitus Varus after Supracondylar Fracture of Humerus in Children

摘　　要：目的探讨一种预防和治疗肱骨髁上骨折以及并发肘内翻的临床疗效。方法将纳入患者分为肱骨髁上骨折组(A组)及骨折后肘内翻畸形组(B组),采用自行设计的双克氏针交叉外翻张力固定方法治疗。术后随访并评价疗效。结果术后骨折及截骨均愈合良好,肘关节功能恢复满意,A组肘内翻12例,发生率为3.9%,总优良率为88.9%;B组肘内翻复发2例,复发率为3.2%,总优良率为94%。结论克氏针交叉外翻张力固定是预防和治疗小儿肱骨髁上骨折后肘内翻的良好固定方法。Objective To explore the clinical efficacy of a kind of prevention and treatment for supracondylar fracture of humerus and cubitus varus. Methods All of the cases included were divided intosupracondylar fracture of humerus group（ A group）and post -fracture cubitus varus deformity（ B group）. Selfdesigned valgus tension fixture with closed - crossed Kirschner wires was adopted in treatment. Post - operative follow up and efficacy assessment were conducted. Results Postoperative recovery and the healing of fractured bones were good. Elbow joint function was recovered satisfactorily. There were 12 cases of cubitus varus in A group and its incidence was 3.9% and the total curative effective rate was 88.9%. There were 2 cases of eubitus varus in B group and its incidence was 3.2% and the total curative effective rate was 94%. Conclusion Valgns tension fixture with closed -crossed Kirschner wires is the satisfactory fixation in the prevention and treatment of cubitus varus after supracondylar fracture of humerus in children.
