镇痛灵辅助硬膜外阻滞的临床观察

摘　　要：目的观察镇痛灵辅助硬膜外阻滞对呼吸、循环及牵拉反应的影响。方法将２４例ＡＳＡⅠ～Ⅱ级择期行盆腔手术的病人随机分为哌替啶（Ｐ）组和镇痛灵（Ｅ）组。麻醉平面固定后，Ｐ组静注哌替啶５０ｍｇ、氟哌啶２．５ｍｇ；Ｅ组静注镇痛灵１５ｍｇ、氟哌啶２．５ｍｇ。结果两组病人术中均无明显牵拉痛及恶心、呕吐等反应。Ｐ组给药后ＳｐＯ２，ＭＡＰ均明显低于用药前（Ｐ＜０．０５，Ｐ＜０．０１），ＨＲ则明显增快（Ｐ＜０．０５）；Ｅ组给药后ＭＡＰ明显增高（Ｐ＜０．０５、Ｐ＜０．０１），ＨＲ和ＳｐＯ２却无明显改变。结论镇痛灵对呼吸、循环无明显抑制。Objective To observe the effects of eptazocine as an adjuvant during epidural blockade on the respiratory,circulatory functions and visceral reaction.Method Twenty four adult women (ASAⅠ～Ⅱ) operated upon for palvic surgery under epidural blockade were randomly divided into two groups.In pethidine group(12 cases),the adjuvant consisted of pethidine 50mg,droperidol 2.5mg.In eptazocine group (12 cases),the adjuvant consisted of eptazocine 15mg , droperidol 2.5mg.Result There is no different visceral reaction between the two groups.SpO 2 and MAP after administration of the adjuvant were lower significantly than that before in pethidine group(P<0.05,P<0.01),HR was increased remarkably also(P<0.05);HR and SpO 2 were kept stable after the adjuvant in eptazocine group,however MAP was increased remarkably after the administration(P<0.05,P<0.01).Conclusion Eptazocine together with droperidol can be used as an adjuvant during epidural blockade.Respiratory and circulatory depression could be avoided when eptazocine is used instead of pethidine.
