摘　　要：本文对2型糖尿病普遍存在的健康管理模式作了系统的介绍、分析与对比,就以往管理模式中存在的不足以及各流程中存在的关键问题进行探索与思考。在各流程中增加了促进健康执行这个步骤并提出相应的对策和建议。避免了信息收集带来的资源浪费及信息存储不便问题,在认识健康问题中避免了评价指标过于单一,方法不够灵活等问题。This article made systemic introduction, analysis and comparison for the common models of health management for type 2 diabetes, and performed exploration and thinking for the deficiency of the management model and the problems of the process. Meanwhile, the author added one step of promoting health implement, and gave some related suggestions. We avoided some problems, such as resources wasting during selecting information and inconvenient？saving; In the process of evaluation, we also avoided problems of much too simple indicates and non-flexible methods.
