蛋壳技术治疗半椎体畸形双侧椎弓根钉与单纯凸侧固定矫形效果比较

摘　　要：目的探讨半椎体畸形引起的脊柱侧弯患者采用蛋壳技术切除半椎体后,不同固定方式对手术矫形效果的影响以及性别、年龄、侧弯原始角度对其的影响。方法对2006年5月～2009年2月骨科手术治疗的45例半椎体畸形引起的脊柱侧弯患者进行随访观察,记录患者术后侧弯矫正效果,并根据其性别、年龄等分组进行统计学分析。结果双侧椎弓根钉固定较单纯凸侧固定后期矫形效果好,侧弯矫正良好率的差异具有统计学意义(P<0.05);不同年龄、不同性别、不同的侧弯原始角度对单双侧固定之间,侧弯矫正良好率的差异无统计学意义(P>0.05)。结论半椎体畸形脊柱侧弯采用蛋壳技术切除半椎体后双侧椎弓根钉固定较单纯凸侧固定后期矫形效果好;性别、年龄、侧弯原始角度对单双侧椎弓根钉固定效果无明显影响。Objective To analyze the long-term effect of double-pedicle screw and cyrto-pedicle screw fixation with ＂egg-shell＂ technology and the effects of different fixation systems. Methods Forty-five surgery-treated patients with scoliosis caused by semi-vertebral deformity were followed up from May 2006 to Feb 2009. The patients were grouped based on sex and age. Results According to fixation ways, the correction rates for scoliosis were significantly different in different fixation groups（P〈0.05）, but were not significantly different in different sex,age, scoliosis proto-cobb angle groups, respectively（ P〉0.05 ）. Conclusion Donble-pedicle screw fixation has a better effect than cyrto-pedicle screw fixation with ＂egg-shell＂ technology,and sex,age and scoliosis proto-cobb angle have no definite influence.
