摘 要:目的评价正常人脑标本MRI与组织学测量的相关程度,比较老年前期与老年期内嗅区皮层和海马在确定层面的形态学变化。方法非中枢神经系统疾病致死的新鲜人脑标本20例(40~59岁10例,65~87岁10例),经福尔马林固定后行MRI扫描。在斜冠状断面像上,测量经乳头体和前连合层面内嗅区皮层的厚度和面积,以及经外侧膝状体层面海马的面积。然后与MRI层面对应切制脑标本,使用图像分析仪进行上述指标的组织学测量。结果经前连合层面内嗅区皮层厚度MRI测量:左侧(0.31±0.05)cm、右侧(0.32±0.06)cm,组织学测量:左侧(0.23±0.02)cm、右侧(0.22±0.03)cm;内嗅区皮层面积MRI测量:左侧(0.62±0.18)cm2、右侧(0.67±0.15)cm2,组织学测量:左侧(0.27±0.04)cm2、右侧(0.30±0.02)cm2。经乳头体层面内嗅区皮层厚度MRI测量:左侧(0.27±0.05)cm、右侧(0.29±0.06)cm,组织学测量:左侧(0.20±0.02)cm、右侧(0.21±0.03)cm;内嗅区皮层面积MRI测量:左侧(0.47±0.14)cm2、右侧(0.49±0.11)c?Purpose To evaluate the correlation of MRI measurement with histological examination of the entorhinal cortex and hippocampus in normal human postmortem brain, and compare the morphological changes in presenile and senile brains. Methods Twenty presenile (10 cases 40~59 years) and senile (10 cases 65~87years) fresh normal human postmortem brains, without central nervous system diseases were fixed with formalin and scanned by MRI. On the oblique coronal planes that passed through the mammillary bodies, the anterior commissure ,and the lateral geniculate bodies, the thickness and area of entorhinal cortex and the area of hippocampus were measured on the screen. Then, the brains were dissected in planes matching the MR scanning, and the aforementioned indexes were measured by pathology. Results On the plane that includes the anterior commissure, the thickness of entorhinal cortex was 0.31± 0.05cm on the left, and 0.32±0.06cm on the right on MRI, 0.23±0.02cm and 0.22±0.03cm respectively on histologic examination. The area of entorhinal cortex was 0.62±0.18cm 2 on the left ,0.67±0.15cm 2 on the right on MRI and 0.27±0.04cm 2, 0.30±0.02cm 2 on histologic slice. On the mammillary bodies plane the thickness was 0.27±0.05cm on the left,0.29±0.06cm on the right on MRI, 0.20±0.02cm and 0.21± 0.03 cm on histologic slice. The area was 0.47±0.14cm 2on the left, 0.49 ±0.11 cm 2 on the right on MRI, 0.27±0.04 cm 2 and 0.29±0.05cm 2 on histologic slice. The area of hippocampus on the plane including lateral geniculate bodies was 0.56±0.09cm 2 on the left, 0.56±0.10cm 2 on the right on MRI, 0.28±0.05 cm 2 and 0.29± 0.07cm 2 on histologic slice. The correlation coefficient of MRI measurements with histologic examination were 0.59~0.78 ( P <0.05~0.01).The differences between left and right, presenile and senile were insignificant( P >0.05). Conclusion The results suggested that measurement of MRI correlated well with that of histologic examination. MRI is a reliable method
