产妇和胎儿因素对普贝生临床效果的影响

摘　　要：目的探讨我国辽宁地区产妇和胎儿因素对普贝生(地诺前列酮栓)促宫颈成熟及引产效果的影响。方法采用病例对照研究的方法,将孕周超过37周、无引产禁忌证、有阴道分娩适应证的单胎妊娠产妇352例分为2组:252例为应用普贝生引产的实验组,另100例为应用小剂量催产素引产的对照组。选择宫颈成熟程度、产程、最终的分娩方式、对胎儿的影响、新生儿评分等5项指标观察普贝生的临床效果,同时选择产妇年龄及胎儿体质量作为指标观察产妇及胎儿因素对用药效果的影响。结果实验组用药后8 h宫颈成熟程度明显高于对照组,产程快于对照组,而对胎儿在宫内的状态和新生儿评分均无明显影响。产妇年龄及正常范围内的胎儿体质量对普贝生的效果无明显影响。结论普贝生可以有效促进宫颈成熟、缩短产程,对胎儿和新生儿无明显影响,而产妇自身的年龄及胎儿体质量对普贝生的临床效果无明显影响。Objective To explore the influence of propess on cervical ripening from the factors of pregnancy women and fetus in Liaoning province. Methods Using the method of experimental control study, 352 patients who bad carried singleton pregnancies and had reached a gestational age of at least 37 weeks without any vaginal dehvery contmindicafion were divided into two groups：experimental group （n =252） and control group （n = 100）. The change of cervical ripening, birth process, the ultimate delivery mode, the influence on fetus and neonatal score were observed as the clinical evaluation after the treatment. The maternal age and weight of fetus were both obsevered. Results Eight hours after administration, the degree of cervical ripening in experimental group was higher than in control group;birth process and the rate of caesarean section were lower than control group. Propess had no influence on the fetus in the uterus. There was no relationship between clinical evaluation of propess and maternal age and the weight of fetus within the normal range. Conclusion Propess can promote cervical ripening, shorten birth process. Propess has no significant impact on the fetus and neonatus, and the maternal age and the weight of fetus had no effect on its clinical evaluation.
