健胃愈疡片干预PU黏膜IL-1的表达及其抗PU复发的机理探讨

摘　　要：目的:观察健胃愈疡片(太子参、陈皮、山药、麦芽和山楂)对溃疡黏膜白细胞介素-1(IL-1)表达的变化,并分析其可能的作用机制。方法:选取消化性溃疡(Peptic Ulcer,PU)患者36例。另设正常对照组12例。胃镜下活检胃黏膜组织,免疫组化检测IL-1的表达,并同时检测氨基己糖及磷脂含量。结果:健胃愈疡片组氨基己糖、磷脂含量增高,健胃愈疡片组治疗后与治疗前比较差异有统计学意义(P<0.01)。健胃愈疡片组PU患者氨基己糖、磷脂含量和IL-1的表达有直线相关关系(P<0.01)。结论:健胃愈疡片通过下调IL-1的表达,影响胃黏膜氨基己糖及磷脂含量,进而影响胃黏膜疏水性,加强黏液凝胶层稳定性,从而防止溃疡的产生和复发,这可能是其促进溃疡愈合的主要作用机制之一。AIM： To observe the influences of Jianwei Yuyang Pills （Radix pseudostellariae, Pericarpium citri reticulatae, Rhizoma dioscoreae, Fructus hordei germinatus, and Fructus crataegi ） on the recurrence of peptic ulcer （ PU）, and the expressions of IL-1 and analyze its potential mechanism. METHODS ： Thirty-six patients with gas- tric ulcer were chosen as Jianwei Yuyang Pills treatment group. In addition, the normal control group was set up （12 cases）. The biospy specimens of gastric mucosa was detected to observe the change in immunohischemistry of IL-1 and in the contents of ninohexose and phosphatide of ulcerated gastric mucosa. RESULTS： The contents of ninohexose, phosphatide of PU patients increased in Jianwei Yuyang Pills group. There was a statistical significance between Jianwei Yuyang Pills group （before treatment） and Jianwei Yuyang Pills group （after treatment） （P 〈 0.01 ）. There was a linear correlation between the contents of phosphatide, aninohexose and the expression of IL-1 in PU patients（P 〈 0.01 ）. CONCLUSION： Jianwei Yuyang Pills can prevent the occurrence and relapse of ulcer and affect the hydrophobicity of gastic mucosa and strengthen the stability of myxogellayer byelevating the express of IL-1 and effecting the contents of phosphatide and aninohexose, this is maybe one of the mechanisms of Jianwei Yuyang Pills to heal ulcer quickly.
