摘  要:目的探讨糖尿病患者糖化血红蛋白水平与血小板膜GPⅡb/Ⅲa复合物纤维蛋白原受体(PAC-1)和P选择素(CD 62P)的关系及其临床意义。方法采用流式细胞仪三色荧光标记技术来测定45例40岁以下初发的2型糖尿病患者及20名健康对照组的PAC-1、CD 62P的表达率和糖化血红蛋白A1(GHbA1)比率,同时进行治疗前后及与对照组比较。结果初发的2型糖尿病组PAC-1(12.38±3.15%)、CD 62P(8.74±2.01%)、GHbA1(9.53±2.73%),其表达分别显著高于健康对照组的2.67±1.52%(P<0.001)、1.07±1.41%(P<0.001)、6.21±1.05%(P<0.001)。降糖治疗6月后糖尿病组血糖达标,PAC-1(7.38±3.25%)、CD 62P(6.26±3.01%)、GHbA1(6.33±1.46%),与治疗前比较有显著差异(P<0.001)。结论2型糖尿病血小板活化与糖化血红蛋白水平密切相关。严格控制血糖可以抑制血小板活化,预防血栓性疾病的发生。Objective To investigate the relation and clinical significance of platelet membrane glucoprotein Ⅱ b/Ⅲa complex fibrinogen receptor (PAC-1 ), CD 62P expression and glucosylated haemoglobin on platelet activation function in patients with type 2 diabetes mellitus. Methods Platelet PAC- 1, CD 62P expression were analyzed by flow cytometry (FCM) and their glucosylated haemoglobin ( GHbA1 ) was determinedin in 45 diabetes mellitus patients ,and compared with patients group,itself group of the after trentment and normal controls. Results The positive rates of PAC- 1 ( 12.38 ± 3.15% ), CD 62P(8.74 ± 2.01% ) and the rate of GHbA1 (9.53 ± 2.73 % ) in 35diabetes mellitus patients were significantly higher than itself group of the after trentment ( 7.38 ± 3.25 % ) ( P 〈 0. 001 ), ( 6.26± 3.01% ) ( P 〈 0.001 ), ( 6.33 ± 1.46 % ) ( P 〈0.010) ;and the normal controls (2.67± 1.52% )(P〈0.001) ,(1.07±1.41% )(P〈0.001) ,( 6.21 ±1.05 % ) ( P 〈 0. 001 ). Conclusion Platelet activation function in type2 diabetes mellitus is related with glucosylated haemoglobin ,strict controlled glueosylated haemoglobin can restain platelet activation, and can prevent thrombosis disease.
