术前心理干预复合音乐疗法对全麻下妇科手术患者心理应激反应的影响

摘　　要：目的：探讨术前心理干预复合音乐疗法对全麻下妇科手术患者心理应激反应的影响。方法：选择ASAⅠ-Ⅱ级妇科手术患者120例,随机分为对照组、心理干预组、音乐组和复合组各30例。在入院后（T0）、静脉穿刺后5 m in（T1）、实验结束即刻（T2）抽取血样,检测血浆皮质醇（Cor）、去甲肾上腺素（NE）和肾上腺素（E）浓度。结果：与对照组相比,心理干预组与复合组在T1、T2时的Cor、E、NE均降低（P〈0.05）,音乐组在T2时E、NE降低（P〈0.05）;复合组各指标在T1、T2时均比音乐组降低（P〈0.05）。结论：心理干预复合个体化音乐疗法是降低患者术前心理应激反应较为有效的方法,值得临床推广应用。Objective： To evaluate the effect of preoperative psychological intervention combined with music therapy on the psychological stress of patients with gynecologic surgery under general anesthesia.Methods： 120 patients（ASA Ⅰ ~ Ⅱ grade） who would have gynecologic surgery were randomly divided into control group,psychological intervention group,music therapy group and the combined intervention group（30 cases for each group）.The blood samples of the patients were collected after admission（T0）,5 minutes after venous puncture [ T1 ） and at the end of the trial （ T2 ） , and the concentration of plasma cortisol （Cor） , norepinephrine （NE） and epinephrine （E） was determined. Results： Compared with the results in control group, the concentration of Cor, E and NE was lower at T1 and T2 in psychological intervention group and combined intervention group （P 〈 0. 05） ; E and NE were lower at T2 in music therapy group （ P 〈 0. 05 ） ; the concentration of all items at T1 and T2 was lower in combined intervention group than that in music therapy group （ P 〈 0. 05 ）. Conclusion ： Psychological intervention combined with music therapy is an effective approach to relieve psychological stress of the patients with gynecologic surgery and it is worthy of clinical application.
