米索前列醇用于中、晚期妊娠引产对胎盘的影响

摘　　要：为了评价米索前列醇阴道给药用于中、晚期妊娠（简称中孕及晚孕）促宫颈成熟及引产对胎儿的安全性,采用随机双育法,比较试验组与对照组（晚孕对照组用安慰剂,中孕对照组用利凡诺）终止妊娠后胎盘大体解剖、光镜结构变化,观察米索前列醇用于晚孕促宫颈成熟及引产对胎盘结构的影响;同时也观察比较米索前列醇和利凡诺终止中孕时对胎盘结构的影响。结果表明,米索前列醇能较快地促进宫颈成熟并在短时间促发临产,也可较快地致中孕引产而分娩有生机儿。所娩出胎盘胎膜完整,胎盘组织无明显出血灶。光镜观察,该药不致绒毛水肿、滋养细胞及蜕膜组织变性坏死,绒毛无明显结节形成细胞出芽。但见绒毛干及绒毛血管不同程度扩张、绒毛膜板有不同程度炎性细胞浸润,此种改变属正常生理反应及（或）良性药理效应.不影响胎盘生理功能,对胎儿是安全的。To evaluate the safety of intravaginal misoprostol on fetus for ripeness of cervix and labor of induction in second and third trimester of pregnancy,with a randomized and doubleblind trial,the gross and microscopic structure of the labored placenta in misoprostol administration was compared with the placenta in control groups (given placebo in control group of third trimester of pregnancy, given revanol in the control group of second trimester). Effect of misoprostol on placental anatomicopathological structure was observed. The results showed that misoprostol rapidly ripened the cervix,induced the onset of labor and made the woman in the second trimester of pregnancy deliver viable infant without affecting the anatomicopathological structure of the placenta. The placenta in misoprostol administration was complete with the whole membranes and had not hemorrhage of villi. The microphase of the placenta did not present edema of villi,denaturation and necrosis of trophoblast and decidua,formation of trophoblast knots,and trophoblast germination,except for the presence of vasodilatation and the leukocytic infiltration. The vasodilatation and leukocytic infiltration were normally physiological changes and benign pharmacological effect without affecting the function of placenta. The change had no damage to the fetus.
