摘  要:目的探讨磷脂酶A2(PLA2)及细胞因子(CKs)在肺炎恶化进程中的作用及意义。方法结合危重病评分.采用ELISA检测25例肺炎患儿血清PLA_2、CKs如肿瘤坏死因子(TNF-α)叶及白细胞介素8(IL-8)含量,并监测患儿脏器功能、血乳酸及动脉血气。结果肺炎组患儿血清PLA2、TNF-α及IL-8含量明显低于伴呼衰组(P<0.05)及伴多衰组(PMO.01);PLA2与TNF-α、IL-8及血乳酸是正相关。结论PLA2及CKS相互作用,参与肺炎的病理生理过程;PLA2与肺炎病情有关,并可作为早期警示呼衰及多衰发生的实验性参数。To investigate the role and significance of phospholipase A_2 (PLA2) and cytokines (CKs ) in the pejorative process ofpneumonia.Methods Combined with the critical care scores, serum concentrations of PLA2, CKs, including tumor necrosis factor (TNF-α)and interleukine (IL-8) were determined in 25 children with pneumonia and 20 healthy children by ELISA method,and the arterial bloodgases, lactic acid (LA) were measUled while the organ functions weresinlultaneousely evaluated.Results Serum concentrations of PLA_2, TNF-α and IL-8 inchildren with pneumonia were significantly lower than those in children with respiratory failure (RF. P< 0. 05 ) and multiple organ failure (MOF. P<0. 01 ); Serum PLA2 levels were positively related toserum TNF-α. IL- 8 and LA. respectively.Conclusions PLA2 and CKs are involved in the pathophysiologicprocess of pneumonia. PLA2 are a laboratory parameter to alerm early to the development of RF and MOF. and are related with thestages of pneumonia.
