摘 要:目的:探讨一侧心室压力负荷时,对对侧心室功能及结构的影响。方法:健康杂种犬7只,环缩肺动脉和主动脉,造成急性右心室和左心室压力负荷的动物模型,并应用心导管术观察血液动力学、应用超声心动图观察心室形态改变。结果:环缩肺动脉后,右心室压力明显升高,左心室压力降低,跨隔压力阶差缩小,舒张末期左心室偏心指数增大(由对照值由102±013增至188±058,P<001)。环缩主动脉后,右心室压力随左心室压力的升高而增高。环缩肺动脉和主动脉后右心室射血分数较环缩前均明显降低(P均<001)。跨隔压力阶差与偏心指数呈显著负相关(r=-0089,P=0.016)。Objective:To evaluate the function and configuration changes of the ventricle under pressure overload of the contralateral ventricle. Methods:Acute ventricular pressure overload model was established by sudden constriction of the pulmonary artery and aortic artery in seven anesthetized open chest dogs.The hemodynamics and the configuration changes of both ventricles were assessed by heart catheterization and echocardiography. Results:During the constriction of the pulmonary artery the end systolic and end diastolic pressure of the right ventricle increased significantly,at the same time that of the left ventricle and transseptal pressure gradient decreased.The end diastolic eccentricity index increased(1.02±0 13 vs.1.88±0.58, p <0 01).During the constriction of the aortic artery,the end systolic and end diastolic pressure of the left ventricle increased dramatically that resulted in a large increase in transseptal pressure gradient.During both the constriction of the pulmonary artery and the constriction of the aortic artery,the right ventricular ejection fraction decreased ( P <0 01).The diastolic transseptal pressure gradient correlated inversely with the eccentric index( r=-0.089,p=0.016 ). Conclusions:The pressure overload of one ventricle can effect the function and configuration of the contralateral one.
