双水平气道正压治疗慢性阻塞性肺病稳定期疗效观察

摘  要:目的探讨家庭型无创机械通气治疗COPD合并慢性2型呼吸衰竭稳定期的疗效及对COPD患者氧化应激的影响。方法选择接受BiPAP无创机械通气并携带家庭型呼吸机返家的COPD并2型呼衰病例13例作为治疗组,以同期同等病情住院期间接受BiPAP机械通气且出院后氧气治疗病例14例作为对照组。观察出院前和出院后6个月两组血气分析,血清超氧化物岐化酶(SOD)、丙二醛(MDA)值。结果治疗组与对照组出院前血气指标、SOD、MDA,两组比较无统计学意义(P>0.05),出院后6个月血气指标,治疗组较对照组明显改善,差异有统计学意义(P<0.01),SOD值治疗组较对照升高,MDA值测定治疗组较对照组降低,差异均有统计学意义(P<0.01)。结论家庭用无创BiPAP机械通气是一项积极有效的治疗稳定期COPD合并2型呼吸衰竭的措施。Objective To investigate the effect of BiPAP to home on COPD with hypercapnie respiratory failure during stable phase as well as the effect on oxidative of organism. Methods 27 inpatients with COPD and hypereapnic respiratory who conducted by noninvasive ventilation via facialmask were enrolled. After discharged from hospital, noninvasive ventilation to home was used in 13 cases (the study group), meanwhile another 14 eases with similar clinical characteristics received oxygen inspiration (the control group ). The blood gas analysis and SOD,MDA value of serum were observed before discharge and six months after discharge. Results Patients in the study group and the control group had similar blood gas analysis and SOD 、MDA value of serum prior to discharge. Six months after discharge, the arterial partial pressure of carbon dioxide more reduced and the PaO2 more improved in the study group than those in the control group (P 〈 0.01 ,P 〈 0.01, respectively). The SOD value of serum more increased and the MDA value of serum more decreased in the study group than those in the control group(P 〈 0.01,P 〈 0.01, respectively). Conclusion Home non -invasive positive pressure ventilation has benefit on COPD patients with hypercapnic respiratory failure during stable phase.
