猪肺磷脂注射液多方位给药防治新生儿呼吸窘迫综合征的临床护理研究

摘　　要：目的:探讨新生儿呼吸窘迫综合征(NRDS)的综合诊治与护理的经验教训,探讨提高其预后的措施。方法:对我院近3年新生儿重症监护病房(NICU)临床诊治及采用综合护理的81例新生儿肺透明膜病患者进行分析。均给予对症支持治疗、氧疗,全面监护等细致的护理措施,依据血氧饱和度、血气检测调整给氧方式。其中头罩吸氧13例,鼻塞式持续正压通气(CPAP)给氧25例,其中16例因病情无改善,改用机械通气治疗。初诊胸片改变为Ⅲ、Ⅳ级者43例直接进行机械通气治疗,应用肺泡表面活性物质(PS)猪肺磷脂注射液治疗37例。结果:81例新生儿HMD治愈65例(80.24%),放弃治疗6例(7.41%),死亡10例(12.35%)。治疗组死亡率、机械通气使用率及机械通气时间显著缩短(t=42.911 P<0.01);结论:细致的护理措施,气管内周到均匀的给予猪肺磷脂注射液可以降低NRDS死亡率,缩短机械通气时间。Objective：To expore the comprehensive treatment and cuteness of neonatal respiratory distress syndrome （NRDS） to find the ways improving their prognosis. Methods： analyse 81 patients with NRDS to be treated in comprehensive care clinical diagnosis and intensive careness in neonatal intensive care unit （NICU） of our hospital.all the cases were treated with symptomatic support therapy, oxygen therapy, comprehensive and meticulous nursing care, etc.Then based on oxygen saturation, blood gas analysis to adjust oxygen. 13 cases in which were treated with oxygen hood,and nasal continuous positive airway pressure （CPAP） oxygen in 25 cases, of which 16 cases due to no improvement chosed mechanical ventilation. 43 cases initially diagnosed chest changed on Ⅲ,Ⅳ grade used mechanical ventilation directly;And 37 cases application injection with Pulmonary. surfactan （PS） tung phosphotipid injection. Results： 65 patients of neonatal HMD were cured （80.24%）, 6 cases gave up treatment（7.41%）, and 10 died （12.35%）. The mortality, utilization of mechanical ventilation and mechanical ventilation time in treatment group were significantly shorter than control group（t = 42.911 P 〈0.01）; Conclusion： Th meticulous care-measures and thoughtful uniform injection oflung phospholipid to endotracheal can reduce NRDS mortality, Shorten the duration of mechanical ventilation.
