老年Ⅱ型糖尿病胰岛素抵抗与非酒精性脂肪肝的相关性研究

摘　　要：目的研究老年Ⅱ型糖尿病(T2DM)患者胰岛素抵抗(IR)与非酒精性脂肪肝(NAFL)的相关性。方法根据B超结果,将64例老年T2DM患者分成脂肪肝组32例及非脂肪肝组32例。分别测定两组患者的空腹血糖(FBG)、血脂、空腹胰岛素(FINS)、体重指数(BMI)及胰岛素抵抗指数(HOMA-IR)并进行比较分析。结果T2DM合并脂肪肝组与非脂肪肝组比较,FBG、TG、FINS、BMI、HOMA-IR明显增高,而HDL降低。结论老年T2DM患者NAFL的发生与胰岛素抵抗密切相关。减肥、降脂、改善胰岛素抵抗对防治糖尿病性NAFL具有重要意义。Objctive To investigate the relationship between insulin resistance（IR）and non alcoholic fatty liver（NAFL）in elderly type 2 diabetes mellitus（T2DM）. Methods According to the results of B-ultrasound, 64 elderly patients with T2DM were divided into fatty liver group and non fatty liver group. FBG, TG, TC, FINS, BMI and HOMA-IR of all patients were examined and analyzed. Results FBG, TG, FINS, BMI, HOMA-IR of the fatty liver group were significantly higher than those of the non fatty liver group, while HDL of the fatty liver group was lower than that of the non fatty liver group. Conclusion NAFL has a close relationship with IR in elderly patients with T2DM. Weight loss, decreased lipid and improving IR are of importance to prevent NAFL.
