摘 要:目的比较脉冲1.44μm与1.06μm波长Nd:YAG激光对犬胃壁作用的异同,评价1.44μm激光的组织效应特点。方法比格犬5条,采用同体对照,分别应用1.44μm(实验组)与1.06μm(对照组)脉冲Nd:YAG激光,由单脉冲能量200,500,800 m J,每次照射脉冲数为1,5,10,20,40,80,组成19种剂量(包括1个零剂量指标),对胃后壁黏膜进行活体照射。实验重复5遍。损伤灶切取标本,制作切片,光学观察,计算机成像与测量。结果实验组与对照组,汽化宽度最大值分别为(1863.8±225.0)μm和(576.8±30.8)μm(t=12.672,P=0.000);凝固宽度最大值分别为(885.1±82.9)μm和(1275.8±140.1)μm(t=-5.366,P=0.001);汽化深度最大值分别为(1787.4±93.7)μm和(105.0±16.3)μm(t=39.539,P=0.000),凝固深度最大值分别为(1327.8±86.3)μm和(994.8±130.0)μm(t=4.773,P=0.002)。结论1.44μm激光对犬胃黏膜有汽化与凝固止血作用,其汽化能力远大于1.06μm激光,其凝固作用稍弱于1.06μm激光,但足以保障实验创面的清洁与止血效果。Objective To compare the histological effects of 1. 44-μm and 1.06-μm pulse Nd: YAG laser radiation on stomach wall tissues in dogs. Methods Five beagle dogs were enrolled in this study to received 1.44-μm (experiment group) and 1.06-μm (self-control group) pulse Nd:YAG laser radiation at the posterior wall of the stomach. Totally 19 dosage of radiation was used including a 0-mJ radiation. The dosage of every single pulse was 200, 500, or 800 mJ, and the amount of the pulses was 1, 5, 10, 20, 40 or 80. The experiment was repeated for 5 times. Afterwards, the injured tissues was obtained and observed under a light microscope and the photomicrograph was taken at the meantime. Results In experimental groups, the widest and photoevaporated zone and coagulation zone were ( 1863. 8 ±225.0) μm and (885. 1 ± 82. 9 ) μm, and the deepest photoevaporated zone and coagulation zone were ( 1787.4 ± 93.7) μm and ( 1327.8 ± 86.3 ) μm, respectively ; which were significantly higher than those in the control [ widest and photoevaporated zone and coagulation zone : (576.8±30.8 ) μm and ( 1275.8 ±140.1 ) μm, t = 12. 672, P = 0. 000 and t = - 5. 366, P = 0. 001 ; deepest photoevaporated zone and coagulation zone : ( 105.0± 16.3) μm and (994.8 ± 130.0) μm, t = 39. 539, P = 0. 000 and t = 4. 773, P = 0. 002 ; respectively ]. Conclusions 1.44-μm Nd: YAG laser is suitable for vaporization and coagulation in stomach surgery. It shows higher ability in vaporization but lower ability of coagulation than 1.06-p.m pulse Nd : YAG laser radiation, which,however does not affect the outcomes of wound cleansing and hemostasis.
