崇义县新型农村合作医疗补偿方案运行状况的比较研究

摘　　要：[目的]对崇义县新型农村合作医疗(新农合)补偿方案的运行状况进行比较分析,从而为调整和完善新型农村合作医疗制度的补偿方案提供科学的理论依据。[方法]采用文献研究和现场调查等方法收集2004～2006年崇义县新农合的相关数据,并对农民的参合情况、基金运行情况、参合农民的受益情况、住院病人和医药费用的分布等进行纵向比较分析。[结果]崇义县2004～2006年3年的参合率均较高,达到77%以上;基金运行状况良好,基金结余率呈现逐年降低趋势;受益率逐年加大,而2005年和2006年补偿率较之2004年则有所下调;参合农民享受住院服务更多的集中于县级医院,县级医院住院人数及其医疗费用所占比例呈逐年上升的趋势。[结论]崇义县现行的新农合制度补偿方案基本上切实可行,效果明显,值得借鉴和推广;此外,还应加大对乡镇卫生院的投入,引导农民到乡镇级医疗机构就医,以便合理有效地利用卫生资源。[ Objective ] To analyze operational status of the compensatory schemes of new rural cooperative medical care system（NCMS） in Chongyi county, and provide scientific basis for further adjusting and consummating compensatory schemes of NCMS. [Methods] The data of NCMS of Chongyi county in 2004-2006 were collected by literature review and field investiga- tion methods, and the vertical comparison was Conducted among the 3 years data （including the state of peasant＇s participatinon, fund operation, compensation, distribution of inpatients and medical expenses） of NCMS in Chongyi county. [Results] It showed that the participating rate of rural medical cooperation among the three years （2004-2006） was high, which was above 77% ; the state of fund operation was good and the fund surplus rate was reduced year by year; the benefit rate was increased from the year 2004 to 2006, while the compensation rate in 2005 and 2006 were lower than that of in 2004; Besides, the insured got much more hospitalization in county-level hospitals, and the proportions of patient＇s number and medical expenses in county-level hospital were increased year by year. [Conclusion] It can be concluded that the current compensatory schemes operated in Chongyi county are feasible and beneficial in the local society. It is worth referring and spreading. Besides, the government should take some measures to enlarge the investment in county-level hospitals and to encourage people to use health resources more effectively and reasonably
