摘 要:[目的]探讨心理健康状态的变化对血常规指标的影响及其与人类白细胞抗原(HLA)-DQB1等位基因的关系。[方法]在普通健康体检人群中采用90项症状清单(SCL-90)量表筛选心理健康异常者50人作为实验组,并选取正常者37人作为对照组,实验室检测其血常规指标;HLA-DQB1*02、*03、*04、*05和*06位点分别利用PCR由第2外显子组特异性引物扩增得到,并研究其与血常规各指标之间的关系。[结果]心理健康状态异常组与对照组比较HLA-DQB1*02位点分布频率升高,差异具有统计学意义(P﹤0.05);心理健康状态异常组与对照组比较PLT、PCT升高,MCV、MCH、MCHC降低,差异具有统计学意义(P﹤0.05);HLA-DQB1*02、*03、*04和*05位点分别对不同血常规检测项目具有影响,差异具有统计学意义(P﹤0.05),相对于对照组,心理异常组WBC、LYMPH、MONO、RBC、PLT、PDW升高,而LYMPH%、HB、HCT、MPV降低。[结论]心理健康状态的改变可以引起血常规某些指标的改变并与HLA-DQB1等位基因表型有关。[ Objective ] To explore the effect of mental health on the blood routine index and its correlation with HLA- DQB1 allele. [ Methods] Fifty persons with abnormal mental status for experimental group and 37 normal persons for control group were selected according to SCL-90. Blood routine examination were conducted by auto equipment; HLA-DQB1^*02, ^*03, ^*04. ^*05 and^*06 alleles were individually amplified by polymerase chain reaction (PCR) using exon 2 group-specific primers and the correlation with blood routine was investxgated. [ Results] Statistical analysis showed significant differences in the distribution frequency of HLA--dqb1^*02 (P 〈 0.5) between the two groups. The PLT and PCT in experimental group were significantly higher than those in control group (P 〈 0.5 ), while the MCV, MCH and MCHC in experimental group were significandy lower than those in control group (P〈 0.5). There was significant difference in the effect of the blood routine examination on the HLA-DQB1^*02, ^*03, ^*04, ^*05 between the experimental group and control group (P 〈 0.5). Compared with control group, WBC, LYMPH, MONO, RBC, PLT and PDW were higher and LYMPH%, HB, HCT and MPV were lower in experimental group. [ Conclusion] The changes of mental health status can change some indicators of blood routine examination and this is related with the phenotypes of HLA-DQB1 allele.
